【摘要】：目的:自發(fā)顱內(nèi)低血壓(SIH)是一種由自發(fā)性脊髓腦脊液漏所引起的臨床疾病,目前越來越多地被人們所認識。盡管以低腦脊液壓力為主要特征,部分SIH患者的腦脊液開放壓并不低。在這項研究中,我們旨在評估正常腦脊液開放壓在SIH患者中所占的比例,并初步探討SIH患者的腦脊液開放壓的影響因素。方法:我們回顧了 206例在我院確診的SIH患者,收集并分析其臨床和影像學(xué)資料(包括人口學(xué)信息、體重指數(shù)、癥狀持續(xù)時間和腦部影像學(xué)結(jié)果)。采用單變量分析和多變量分析來識別腦脊液開放壓的潛在影響因素。結(jié)果:本研究納入的206例SIH患者中,共有114(55.3%)例患者的腦脊液開放壓≤60mmH2O,90(43.7%)例患者的腦脊液開放壓在60-200 mmH2O之間,2(1.0%)例患者的腦脊液開放壓200mmH2O。單因素分析表明,癥狀持續(xù)時間(P0.001)、體重指數(shù)(BMI)(P0.001)和年齡(P=0.024)與腦脊液開放壓呈正相關(guān)。然而,多因素分析顯示,僅癥狀持續(xù)時間(P0.001)和BMI(P0.001)與腦脊液開放壓之間有顯著相關(guān)性。同時,多因素分析模型中得出相對較高的R2值(0.681)。結(jié)論:本研究表明,即使未出現(xiàn)腦脊液壓力降低,SIH的診斷不應(yīng)完全排除。BMI和癥狀持續(xù)時間可以影響SIH患者的腦脊液開放壓大小,此外,仍然存在其他潛在因素需要進一步研究。
[Abstract]:Objective: spontaneous intracranial hypotension (SIH) is a clinical disease caused by spontaneous cerebrospinal fluid leakage. Although low cerebrospinal fluid pressure is the main characteristic, cerebrospinal fluid open pressure is not low in some SIH patients. In this study, we evaluated the proportion of open cerebrospinal fluid pressure in patients with SIH, and preliminarily investigated the factors affecting the open pressure of cerebrospinal fluid in patients with SIH. Methods: we reviewed 206 patients with SIH diagnosed in our hospital and collected and analyzed their clinical and imaging data (including demographic information, body mass index (BMI), duration of symptoms and brain imaging results).
