【摘要】：目的:考察左乙拉西坦添加治療老年癲癇患者的臨床效果及安全性。方法:將2014年1月-2016年1月于重慶市云陽(yáng)縣中醫(yī)院就診的老年癲癇患者88例按照隨機(jī)數(shù)字表法分為觀察組與對(duì)照組,各44例。對(duì)照組患者給予卡馬西平+苯巴比妥常規(guī)抗癲癇治療;觀察組患者在對(duì)照組基礎(chǔ)上加用左乙拉西坦片,初始劑量0.25g,后期根據(jù)患者病情可增至0.5g,bid。兩組療程均為6個(gè)月。觀察兩組患者的臨床療效、格拉斯哥昏迷評(píng)分法(GCS)評(píng)分、巴塞爾(Barthel)指數(shù)評(píng)分、蒙特利爾認(rèn)知評(píng)估量表(Mo CA)評(píng)分和不良反應(yīng)發(fā)生情況。結(jié)果:觀察組患者的總有效率、控制率和Mo CA評(píng)分均顯著高于對(duì)照組,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。兩組患者治療后的GCS和Barthel指數(shù)評(píng)分均明顯高于治療前,且觀察組患者的2項(xiàng)評(píng)分均明顯高于對(duì)照組,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。兩組患者均未見(jiàn)明顯不良反應(yīng)發(fā)生。結(jié)論:采用左乙拉西坦添加治療老年癲癇的療效優(yōu)于常規(guī)治療方案,使患者的昏迷程度顯著降低,日常生活能力與認(rèn)知功能顯著提高,且安全性較好。
[Abstract]:Objective: to investigate the clinical effect and safety of levoethylacetam in the treatment of senile epilepsy. Methods: 88 elderly epileptic patients from January 2014 to January 2016 were divided into two groups: observation group (n = 44) and control group (n = 44). The patients in the control group were given carbamazepine phenobarbital routine antiepileptic therapy, and the patients in the observation group were treated with levoethylacetam tablet on the basis of the control group, the initial dose was 0.25 g, and in the later stage, it could be increased to 0.5g bidbidin according to the patient's condition. The course of treatment was 6 months in both groups. The clinical efficacy, Glasgow coma score (GCS), Basel (Barthel) Index (Barthel), Montreal Cognitive Assessment scale (Mo CA) and adverse events were observed. Results: the total effective rate, control rate and Mo CA score in the observation group were significantly higher than those in the control group (P0.05). The GCS and Barthel index scores of the two groups after treatment were significantly higher than those before treatment, and the two scores of the observation group were significantly higher than those of the control group, the difference was statistically significant (P0.05). No significant adverse reactions occurred in both groups. Conclusion: the efficacy of levoethylacetam in the treatment of senile epilepsy is better than that of routine treatment. The degree of coma is significantly reduced, the ability of daily life and cognitive function are improved significantly, and the safety is better.
