【摘要】：目的探索聯(lián)合多個(gè)體檢指標(biāo)建立腦動(dòng)脈硬化預(yù)測模型及評(píng)分系統(tǒng),并評(píng)價(jià)該預(yù)測模型及評(píng)分系統(tǒng)。方法根據(jù)經(jīng)顱多普勒超聲(TCD)結(jié)果將1 325例健康體檢人群分為非腦動(dòng)脈硬化組767例和腦動(dòng)脈硬化組558例。對(duì)其進(jìn)行問卷調(diào)查及收集體檢指標(biāo),使用多因素Logistic回歸模型建立腦動(dòng)脈硬化的預(yù)測模型,進(jìn)一步探索建立腦動(dòng)脈硬化的預(yù)測評(píng)分系統(tǒng)。結(jié)果選出P0.05的指標(biāo)6個(gè)(包括性別、年齡、脈壓、同型半胱氨酸、腰圍、吸煙)進(jìn)行多因素逐步Logistic回歸,建立數(shù)學(xué)模型:Logitistic(PI)=0.232×性別+0.155×吸煙+0.026×年齡+0.005×脈壓+0.005×同型半胱氨酸+0.004×腰圍-1.029。結(jié)合臨床資料,選出性別、高血壓、頸動(dòng)脈硬化、糖尿病、吸煙、年齡、同型半胱氨酸、高敏C反應(yīng)蛋白、脈壓和體重指數(shù)10項(xiàng)指標(biāo)建立腦動(dòng)脈硬化的預(yù)測評(píng)分系統(tǒng),總分為10分。結(jié)論基于常規(guī)體檢指標(biāo)成功建立了腦動(dòng)脈硬化預(yù)測模型和評(píng)分系統(tǒng),值得擴(kuò)展研究。
[Abstract]:Objective to establish a prediction model and scoring system for cerebral arteriosclerosis combined with multiple physical examination indexes, and to evaluate the prediction model and scoring system. Methods according to the results of transcranial Doppler (TCD), 1 325 healthy people were divided into non-cerebral arteriosclerosis group (767 cases) and cerebral arteriosclerosis group (558 cases). Questionnaire survey and physical examination index were collected. The prediction model of cerebral arteriosclerosis was established by using multi-factor Logistic regression model, and the prediction scoring system of cerebral arteriosclerosis was further explored. Results six indexes (including gender, age, pulse pressure, homocysteine, waist circumference, smoking) were selected for stepwise Logistic regression. The mathematical model: Logitistic (PI) = 0.232 脳 sex 0.155 脳 smoking 0.026 脳 age 0.005 脳 pulse pressure 0.005 脳 homocysteine 0.004 脳 waist circumference -1.029 was established. According to the clinical data, 10 indexes including sex, hypertension, carotid atherosclerosis, diabetes, smoking, age, homocysteine, Gao Min C-reactive protein, pulse pressure and body mass index were selected to establish a predictive scoring system for cerebral arteriosclerosis. The total score is 10. Conclusion the prediction model and scoring system of cerebral arteriosclerosis based on routine physical examination are established successfully, which is worthy of extensive study.
