【摘要】：目的探討視神經(jīng)脊髓炎譜系疾病(Neuromyelitis optica disorders,NMOSDs)患者腦脊液蛋白與臨床致殘狀況的關(guān)系及其臨床意義。方法回顧性分析2010年1月~2016年2月我院神經(jīng)內(nèi)科確診的108例NMOSD患者臨床與生化資料,根據(jù)臨床擴(kuò)展致殘量表評(píng)分(Expanded disability status scale,EDSS)將患者分為獨(dú)立行走受限與不受限組,并比較兩組患者間的臨床、生化特征;根據(jù)腦脊液蛋白值將患者分為腦脊液蛋白正常組與異常組,并比較兩組間EDSS評(píng)分的變化;分析腦脊液蛋白與臨床致殘狀況的相關(guān)性。結(jié)果獨(dú)立行走受限組與不受限組之間患者年齡、二便障礙、外周血白細(xì)胞(WBC)、中性粒細(xì)胞比(N%)、腦脊液蛋白、腦脊液Ig G較有統(tǒng)計(jì)學(xué)差異(P0.05);性別、病程、視力損害、AQP4抗體、24 h鞘內(nèi)合成率、寡克隆帶比較無(wú)統(tǒng)計(jì)學(xué)差異(P0.05),兩組不同腦脊液蛋白水平與患者EDSS值比較有統(tǒng)計(jì)學(xué)差異(T=3.13,P=0.002);腦脊液蛋白水平與獨(dú)立行走受限呈正相關(guān)(r=0.286,P0.01)。結(jié)論 NMOSD患者腦脊液蛋白值越高,患者致殘發(fā)生的可能性越大,腦脊液蛋白有助于視神經(jīng)脊髓炎譜系疾病患者致殘狀況的病情評(píng)估。
[Abstract]:Objective to investigate the relationship between cerebrospinal fluid protein (CSF) and clinical disability in patients with optic neuromyelitis (Neuromyelitis optica disorders,NMOSDs) and its clinical significance. Methods the clinical and biochemical data of 108 patients with NMOSD diagnosed by neurology from January 2010 to February 2016 were retrospectively analyzed. According to the clinical extended disability scale (Expanded disability status scale,EDSS), the patients were divided into two groups: independent walking restricted group and non-restricted group. The clinical and biochemical characteristics of the two groups were compared. According to the cerebrospinal fluid protein value, the patients were divided into normal CSF protein group and abnormal cerebrospinal fluid protein group, and the changes of EDSS score between the two groups were compared, and the correlation between CSF protein and clinical disability was analyzed. Results there were significant differences in age, defecation disorder, (WBC), neutrophil ratio (N%), cerebrospinal fluid protein (CSF) and cerebrospinal fluid (Ig G) between the independent walking restricted group and the non-restricted group (P0.05). There was no significant difference in visual impairment, AQP4 antibody, 24 h intrathecal synthesis rate and oligoclonal band (P0.05). There was significant difference between the two groups in cerebrospinal fluid protein level and EDSS value (T = 3.13), and cerebrospinal fluid protein level was positively correlated with independent walk restriction (r = 0.286, P 0.01). Conclusion the higher the cerebrospinal fluid protein value in NMOSD patients, the greater the possibility of disability. CSF protein is helpful to evaluate the disability status of patients with optic neuromyelitis spectrum disease.
