【摘要】：目的:研究不同級(jí)別腦膜瘤的影像特征與MDM2和PTEN表達(dá)的關(guān)系,探究不同級(jí)別腦膜瘤影像表現(xiàn)的分子病理基礎(chǔ),分析不同級(jí)別與MDM2,PTEN蛋白表達(dá)及關(guān)系,兩種蛋白表達(dá)與MRI征象的關(guān)系,進(jìn)而揭示腫瘤發(fā)生發(fā)展中生物學(xué)行為與影像征象的關(guān)聯(lián)。方法:收集蘭州大學(xué)第二醫(yī)院2012年1月1日至2016年9月1日間經(jīng)手術(shù)及病理證實(shí)的病例,術(shù)前均行MRI平掃及增強(qiáng)掃描,盲法記錄各病例MRI特征,查找臘塊進(jìn)行MDM2、PTEN染色;所得數(shù)據(jù)應(yīng)用Spss19.0統(tǒng)計(jì)軟件進(jìn)行統(tǒng)計(jì)學(xué)分析。結(jié)果:不同級(jí)別腦膜瘤MRI分葉征(χ2=32.018,P0.001)、瘤腦界面(χ2=20.292,P=0.001)、瘤周水腫(χ2=8.753,P=0.013)、壞死囊變(χ2=12.619,P=0.002)、強(qiáng)化均勻性(χ2=13.214,P=0.001)、鄰近骨質(zhì)改變(χ2=10.431,P=0.005)6項(xiàng)差異均有統(tǒng)計(jì)學(xué)意義(P均0.05),腦膜尾征(χ2=4.282,P=0.118)、腫瘤強(qiáng)化程度(χ2=5.603,P=0.061)及瘤內(nèi)血管流空信號(hào)(χ2=3.617,P=0.164)3項(xiàng)差異無(wú)統(tǒng)計(jì)學(xué)意義(P均0.05)。腦膜瘤MDM2、PTEN蛋白免疫組化染色表達(dá)陽(yáng)性,MDM2在腦膜瘤不同級(jí)別表達(dá)有差異(H=67.15,P0.001),呈正相關(guān)(r=0.889,P0.001);PTEN在腦膜瘤不同級(jí)別表達(dá)有差異(H=69.59,P0.001),呈負(fù)相關(guān)(r=-0.905,P0.001)。分葉征、瘤腦界面、瘤周水腫、壞死囊變、腫瘤強(qiáng)化后均勻性及鄰近骨質(zhì)改變6項(xiàng)MRI征象與MDM2表達(dá)呈正相關(guān),6項(xiàng)MRI征象與PTEN表達(dá)呈負(fù)相關(guān)。結(jié)論:不同級(jí)別腦膜瘤的MRI特征和MDM2、PTEN表達(dá)存在差異,分葉征象、瘤腦界面模糊、瘤周水腫、壞死囊變、強(qiáng)化不均勻及鄰近顱骨的改變等征象的鑒別意義較大,6項(xiàng)MRI征象與MDM2表達(dá)呈正相關(guān),與PTEN表達(dá)呈負(fù)相關(guān),揭示了不同級(jí)別腦膜瘤MRI征象與MDM2及PTEN表達(dá)相關(guān)性,為提高腦膜瘤分級(jí)診斷、臨床治療和預(yù)后判斷提供依據(jù)。
[Abstract]:Objective: to study the relationship between the imaging features of meningiomas of different grades and the expression of MDM2 and PTEN, to explore the molecular pathological basis of the imaging manifestations of meningiomas of different grades, and to analyze the expression and relationship between MDM2,PTEN protein and different grades of meningiomas. The relationship between the expression of two proteins and MRI signs, and the relationship between biological behavior and imaging signs in tumorigenesis and development were revealed. Methods: cases confirmed by operation and pathology from January 1, 2012 to September 1, 2016 in the second Hospital of Lanzhou University were collected.
