腦膜瘤的MRI征象與MDM2和PTEN因子表達的臨床應用研究

【摘要】:目的:研究不同級別腦膜瘤的影像特征與MDM2和PTEN表達的關系,探究不同級別腦膜瘤影像表現(xiàn)的分子病理基礎,分析不同級別與MDM2,PTEN蛋白表達及關系,兩種蛋白表達與MRI征象的關系,進而揭示腫瘤發(fā)生發(fā)展中生物學行為與影像征象的關聯(lián)。方法:收集蘭州大學第二醫(yī)院2012年1月1日至2016年9月1日間經手術及病理證實的病例,術前均行MRI平掃及增強掃描,盲法記錄各病例MRI特征,查找臘塊進行MDM2、PTEN染色;所得數(shù)據應用Spss19.0統(tǒng)計軟件進行統(tǒng)計學分析。結果:不同級別腦膜瘤MRI分葉征(χ2=32.018,P0.001)、瘤腦界面(χ2=20.292,P=0.001)、瘤周水腫(χ2=8.753,P=0.013)、壞死囊變(χ2=12.619,P=0.002)、強化均勻性(χ2=13.214,P=0.001)、鄰近骨質改變(χ2=10.431,P=0.005)6項差異均有統(tǒng)計學意義(P均0.05),腦膜尾征(χ2=4.282,P=0.118)、腫瘤強化程度(χ2=5.603,P=0.061)及瘤內血管流空信號(χ2=3.617,P=0.164)3項差異無統(tǒng)計學意義(P均0.05)。腦膜瘤MDM2、PTEN蛋白免疫組化染色表達陽性,MDM2在腦膜瘤不同級別表達有差異(H=67.15,P0.001),呈正相關(r=0.889,P0.001);PTEN在腦膜瘤不同級別表達有差異(H=69.59,P0.001),呈負相關(r=-0.905,P0.001)。分葉征、瘤腦界面、瘤周水腫、壞死囊變、腫瘤強化后均勻性及鄰近骨質改變6項MRI征象與MDM2表達呈正相關,6項MRI征象與PTEN表達呈負相關。結論:不同級別腦膜瘤的MRI特征和MDM2、PTEN表達存在差異,分葉征象、瘤腦界面模糊、瘤周水腫、壞死囊變、強化不均勻及鄰近顱骨的改變等征象的鑒別意義較大,6項MRI征象與MDM2表達呈正相關,與PTEN表達呈負相關,揭示了不同級別腦膜瘤MRI征象與MDM2及PTEN表達相關性,為提高腦膜瘤分級診斷、臨床治療和預后判斷提供依據。
