【摘要】：目的探討兒童創(chuàng)傷性腦梗死恢復(fù)的影響因素以及死亡的危險(xiǎn)因素。方法對(duì)66例創(chuàng)傷性腦梗死患兒的臨床資料進(jìn)行回顧性分析。結(jié)果 66例創(chuàng)傷性腦梗死患兒中,死亡12例(18.2%),存活54例(81.8%),包括植物人狀態(tài)6例(9.1%)。54例創(chuàng)傷性腦梗死存活患兒恢復(fù)時(shí)間的影響因素為大面積腦梗死、感染、復(fù)合傷(OR=2.903,OR=2.092,OR=2.859;均P0.05)。與創(chuàng)傷性腦梗死存活患兒比較,死亡患兒入院時(shí)GCS評(píng)分明顯降低,大面積腦梗死、復(fù)合傷、低血壓或休克、腦疝、癲癇的比例明顯增加,脫水藥物的使用尤其是大劑量使用的比例明顯增加(均P0.05)。多因素Logistic回歸分析發(fā)現(xiàn),創(chuàng)傷性腦梗死患兒死亡的獨(dú)立危險(xiǎn)因素分別為入院時(shí)GCS評(píng)分降低、復(fù)合傷、低血壓或休克(均P0.05)。結(jié)論影響兒童創(chuàng)傷性腦梗死恢復(fù)時(shí)間的因素為大面積腦梗死、感染、復(fù)合傷。入院時(shí)GCS評(píng)分低、復(fù)合傷、低血壓或休克是兒童創(chuàng)傷性腦梗死死亡的獨(dú)立危險(xiǎn)因素。
[Abstract]:Objective to investigate the influencing factors and risk factors of death in children with traumatic cerebral infarction. Methods the clinical data of 66 children with traumatic cerebral infarction were retrospectively analyzed. Results among 66 children with traumatic cerebral infarction, 12 (18.2%) died and 54 (81.8%) survived. Among them, 6 cases (9.1%) of vegetative state and 54 cases of survival time of traumatic cerebral infarction were affected by large area cerebral infarction, infection and complex injury (OR2.903OR2.092OR2.859; all P 0.05). Compared with the surviving children with traumatic cerebral infarction, the GCS scores of the dead children were significantly lower at admission, and the proportion of large area cerebral infarction, complex injury, hypotension or shock, cerebral hernia and epilepsy were significantly increased. The proportion of dehydrated drugs used, especially in large doses, increased significantly (P0.05). Multivariate logistic regression analysis showed that the independent risk factors of death in children with traumatic cerebral infarction were decreased GCS score on admission, combined injury, hypotension or shock (P0.05). Conclusion the factors influencing the recovery time of traumatic cerebral infarction in children are large area cerebral infarction, infection and complex injury. Low GCS score, combined injury, hypotension or shock were independent risk factors for death in children with traumatic cerebral infarction.
