【摘要】：目的:探討維持性血液透析(maintenance hemodialysis,MHD)患者中不寧腿綜合征(restless legs syndrome,RLS)的發(fā)病特點(diǎn)及相關(guān)因素,為該病的防治提供科學(xué)依據(jù)。方法:調(diào)查并收集重慶醫(yī)科大學(xué)附屬第一醫(yī)院血液凈化中心MHD患者的一般臨床資料及生化指標(biāo),根據(jù)國(guó)際不寧腿綜合征研究組2012年公布的診斷標(biāo)準(zhǔn)對(duì)患者進(jìn)行RLS的診斷及病情評(píng)分,比較RLS與非RLS患者各指標(biāo)的差異,并通過Logistic回歸分析評(píng)估影響MHD患者發(fā)生RLS的相關(guān)因素。結(jié)果:307例MHD患者中有37例合并RLS,發(fā)生率12.1%,其中73.0%為輕、中度RLS,21.6%為重度RLS,5.4%為極重度RLS;在RLS患者中83.8%為慢性,16.2%為急性或亞急性。RLS與非RLS兩組患者年齡、性別、透析齡、血壓、體重指數(shù)、原發(fā)病構(gòu)成、尿素清除指數(shù)(Kt/V)、日均尿量、煙酒史、血紅蛋白、肌酐、尿素、尿酸、血鈣、血磷、血鎂、血鉀、全段甲狀旁腺素、前白蛋白、白蛋白和堿性磷酸酶等差異均無統(tǒng)計(jì)學(xué)意義(P0.05),但RLS組患者日常鍛煉頻率顯著低于非RLS組患者(Z=-4.114,P0.001)。Logistic回歸分析顯示日常鍛煉情況與MHD患者RLS的發(fā)生呈顯著負(fù)相關(guān)(B=-2.203,OR=0.111,95%CI:0.033~0.371,P0.001)。結(jié)論:RLS是MHD患者的常見并發(fā)癥,在臨床上以慢性和輕、中度RLS為主,長(zhǎng)期缺乏規(guī)律運(yùn)動(dòng)可能是RLS發(fā)生的重要危險(xiǎn)因素。
[Abstract]:Objective: to investigate the characteristics and related factors of restless legs syndrome in maintenance hemodialysis patients with maintenance hemodialysis (MHD), and to provide scientific basis for the prevention and treatment of the disease. Methods: the general clinical data and biochemical indexes of MHD patients in the blood purification center of the first affiliated Hospital of Chongqing Medical University were investigated and collected. According to the diagnostic criteria published by the international restless leg syndrome research group in 2012, the RLS was diagnosed and evaluated. The differences between RLS and non-RLS were compared, and the related factors affecting the occurrence of RLS in MHD patients were evaluated by logistic regression analysis. Results among 307 MHD patients, 37 cases were associated with RLSs, with an incidence rate of 12.1, of which 73.0% were mild and 21.6% were severe RLSs, 83.8% were chronic RLSs, 16.2% were acute or subacute, and 16.2% were acute or subacute. RLS and non-RLS patients were age, sex, dialysis age, blood pressure, hemodialysis age, blood pressure, Body mass index, primary constitution, urea clearance index KT / V, daily urine volume, history of tobacco and alcohol, hemoglobin, creatinine, urea, uric acid, serum calcium, serum phosphorus, serum magnesium, blood potassium, whole segment parathyroid hormone, prealbumin, There was no significant difference in albumin and alkaline phosphatase, but the frequency of daily exercise in RLS group was significantly lower than that in non-RLS group. Logistic regression analysis showed that there was a significant negative correlation between daily exercise and the occurrence of RLS in MHD patients. Conclusion: RLS is a common complication in MHD patients. Chronic, mild and moderate RLS is the main clinical risk factor for RLS. The long-term lack of regular exercise may be an important risk factor for the occurrence of RLS.
