【摘要】：目的:通過對我國六省市農(nóng)村地區(qū)≥40歲人群腦卒中高危人群篩查數(shù)據(jù)分析,探討我國農(nóng)村地區(qū)腦卒中患病情況及相關(guān)危險因素,為制定農(nóng)村腦卒中防治策略,開展腦卒中危險因素干預(yù)提供依據(jù)。方法:本文所用數(shù)據(jù)來自國家人口健康科學(xué)數(shù)據(jù)共享平臺中國卒中數(shù)據(jù)中心,是國家衛(wèi)生計生委新醫(yī)改重大專項《腦卒中高危人群篩查與防治工程》數(shù)據(jù)資源。數(shù)據(jù)來自2012年我國六省市農(nóng)村地區(qū)≥40歲384272例篩查對象。研究上述六省市農(nóng)村地區(qū)居民腦卒中患病、危險分層及主要危險因素分布情況。結(jié)果:1、人群腦卒中患病情況:北京市、河南省、山東省、四川省、山西省、陜西省六省市農(nóng)村地區(qū)40歲及以上人群中腦卒中患者共8196例,檢出率為2. 13%。其中男性4274例,檢出率為2. 39%;女性3922例,檢出率為1.95%。男性高于女性,差異有統(tǒng)計學(xué)意義,P0. 001。2、腦卒中患病地域分析:在北京市、河南省、山東省、四川省、山西省、陜西省六省市農(nóng)村地區(qū)384272例篩查人群中,腦卒中檢出率河南最高,為3. 56%,此后依次為北京市2.95%,山東省2.73%,山西省2.33%,陜西省1.29%,四川省0.63%。篩查人群的腦卒中患病存在地區(qū)差異。3、腦卒中患病年齡段分析:8196例腦卒中患者中,40～64歲的患者有4387例,占53.53%。隨著年齡增大,腦卒中檢出率增高。55歲以上人群腦卒中檢出率上升到接近人群腦卒中檢出率,以后年齡組檢出率呈大幅度上升,各年齡組間腦卒中患病差異有統(tǒng)計學(xué)意義,P0. 001。4、腦卒中風(fēng)險評估:我國六省市農(nóng)村地區(qū)384272例篩查人群中,腦卒中低危人群 277928 例(72. 33%),中危人群 51622 例(13. 43%),高危人群 54722 例(14. 24%)。5、人群腦卒中危險因素分析:我國六省市農(nóng)村地區(qū)≥40歲人群中腦卒中危險因素血脂異常暴露率38. 47%最高,其次依次是吸煙(21.49%)、高血壓(20. 77%)、明顯超重或肥胖(17. 03%)、體育鍛煉少(16. 64%)、腦卒中家族史(7.00%)、糖尿病(4.08%)、房顫或瓣膜性心臟病(2.21%)。男性腦卒中危險因素暴露率前幾位依次為:吸煙(43.83%)、血脂異常(37. 44%)、高血壓(18.46%)、體育鍛煉少(15. 21%)、明顯超重或肥胖(13.59%)。女性腦卒中危險因素暴露率排前幾位的依次為:血脂異常(39. 37%)、高血壓(22. 78%)、明顯超重或肥胖(20. 02%)。吸煙的暴露率則為男性明顯高于女性,而高血壓、血脂異常、明顯超重或肥胖、體育鍛煉少、有腦卒中家族史、糖尿病、房顫或瓣膜性心臟病等的暴露率均為女性高于男性,差異均有統(tǒng)計學(xué)意義(P均0.001)。結(jié)論:1、我國六省市農(nóng)村地區(qū)≥40歲人群中腦卒中患者檢出率為2.13%。其中男性檢出率2. 39%,高于女性(1.95%)。2、我國六省市農(nóng)村地區(qū)≥40歲人群中腦卒中檢出率河南最高,為3.56%,其后依次為北京市2. 95%,山東省2. 73%,山西省2. 33%,陜西省1.29%,四川省0. 63%。3、我國六省市農(nóng)村地區(qū)≥40歲人群腦卒中患者中,40-64歲患者占53.53%。嚴(yán)重影響農(nóng)村勞動力人群的健康和生活質(zhì)量。農(nóng)村中年人腦卒中篩查與防控應(yīng)引起高度重視。4、我國六省市農(nóng)村地區(qū)≥40歲篩查人群中腦卒中低危個體占72. 33%,中危個體占13. 43%,高危個體占14. 24%。5、我國六省市農(nóng)村地區(qū)≥40歲人群中腦卒中危險因素血脂異常暴露率38.47%最高,其后依次是吸煙、高血壓、明顯超重或肥胖、體育鍛煉少等。
[Abstract]:Objective: To explore the incidence of stroke and related risk factors in rural areas of our country by analyzing the screening data of the high risk population of cerebral apoplexy in the rural areas of six provinces and cities in China, and to provide the basis for the prevention and treatment of stroke risk factors in rural areas. Methods: the data used in this paper are from the national population health department. The data sharing platform of Chinese stroke data center is the data resource of the National Health Planning Commission's major new medical reform project, the screening and prevention project of high risk population for stroke. The data came from the screening objects of 384272 cases in rural areas of six provinces and cities over 40 years old in 2012.
