【摘要】：目的:本研究基于國際最新的衛(wèi)生費用核算體系2011版(SHA 2011)核算2014年吉林省老年人心腦血管疾病治療費用,并分析其籌資來源、費用流向等,為合理控制老年人心腦血管疾病費用負擔提供科學依據(jù)。方法:基于SHA 2011框架核算2014年吉林省老年人心腦血管疾病費用總量、籌資方案、機構流向、人群費用分布等。結果:2014年吉林省老年人心腦血管疾病治療費用為34.89億元,占全人群心腦血管疾病費用的70.46%;費用最高的前五位疾病依次為其他類別的心臟病、動脈粥樣硬化、腦梗死、出血性腦血管病和原發(fā)性高血壓;80~84歲年齡組心腦血管疾病治療費用占比最高,達到22.55%;家庭衛(wèi)生支出占比為48.87%,略高于公共籌資占比;發(fā)生在醫(yī)院的老年人心腦血管疾病治療費用占86.64%;老年人心腦血管疾病治療費用中男性高于女性。結論:吉林省老年人心腦血管疾病費用個人負擔較重,費用機構流向不合理,仍需加強對老年人群心腦血管疾病的重點防控,進一步完善老年人醫(yī)療保障制度。
[Abstract]:Objective: to calculate the cost of cardiovascular and cerebrovascular diseases in Jilin province in 2014 based on the latest international health cost accounting system (SHA2011, 2011), and to analyze the source of financing and the direction of expenditure, etc. To provide a scientific basis for reasonable control of the burden of cardiovascular and cerebrovascular diseases in the elderly. Methods: based on the framework of SHA 2011, the total cost of cardiovascular and cerebrovascular diseases, the financing scheme, the direction of organization and the distribution of population cost were calculated in Jilin province in 2014. Results: the cost of treating cardiovascular and cerebrovascular diseases in the elderly in Jilin Province in 2014 was 3.489 billion yuan, accounting for 70.46% of the total cost of cardiovascular and cerebrovascular diseases, and the top five diseases of the highest expense were heart disease, atherosclerosis and cerebral infarction. The treatment cost of cardiovascular and cerebrovascular diseases was the highest (22.55) in the age group of 80 years old with hemorrhagic cerebrovascular disease and essential hypertension, and the proportion of family health expenditure was 48.87, which was slightly higher than that of public financing. The cost of treating cardiovascular and cerebrovascular diseases in the elderly was 86.64, and the cost of treating cardiovascular and cerebrovascular diseases in the elderly was higher in males than in females. Conclusion: the individual burden of cardiovascular and cerebrovascular diseases of the elderly in Jilin Province is heavy, and the flow of expenditure organizations is unreasonable. It is still necessary to strengthen the prevention and control of cardiovascular and cerebrovascular diseases among the elderly and to further improve the medical security system for the elderly.
