【摘要】：目的探討不同嚴重程度急性腦梗死患者心肌損傷程度,并分析其預(yù)后。方法 113例腦梗死患者根據(jù)NIHSS評分分為輕度腦梗死組(A組,n=24),中度腦梗死組(B組,n=55)和重度腦梗死組(C組,n=34)。對3組患者一般資料,肌鈣蛋白Ⅰ、肌紅蛋白、肌酸激酶同工酶(CKMB)、心電圖以及預(yù)后情況進行分析。結(jié)果 A、B、C 3組患者高血壓、糖尿病、高膽固醇史等基礎(chǔ)資料差異無統(tǒng)計學(xué)意義(P0.05)。3組患者肌鈣蛋白Ⅰ、肌紅蛋白、CK-MB水平差異顯著(P0.05),均表現(xiàn)為C組最高、A組最低。3組患者心電圖異常發(fā)生率差異顯著(P0.05),C組發(fā)生率最高,B組次之,A組最低。A、B、C 3組患者死亡率分別為4.17%、5.45%、17.65%,差異有統(tǒng)計學(xué)意義(χ~2=4.700,P0.05)。結(jié)論重度腦梗死患者多存在肌鈣蛋白Ⅰ、肌紅蛋白以及CK-MB水平升高以及心電圖異常,預(yù)后不良,死亡率較高。
[Abstract]:Objective To investigate the degree of myocardial injury in patients with acute cerebral infarction and analyze its prognosis . Methods 113 patients with cerebral infarction were divided into mild cerebral infarction group ( group A , n = 24 ) , moderate cerebral infarction group ( group B , n = 55 ) and severe cerebral infarction group ( group C , n = 34 ) according to NIHSS score . Results There was no significant difference in the levels of troponin I , troponin I , CK - MB in group A , group B and C ( P0.05 ) . The rates of ECG abnormalities in group A , B and C were significantly different ( P0.05 ) . Conclusion There is a high level of troponin I , Myoglobin and CK - MB in patients with severe cerebral infarction , abnormal ECG , poor prognosis and high mortality .
