【摘要】：目的探討帕金森伴慢性心力衰竭(簡稱心衰)患者心率變異性(HRV)的變化特點及其與心功能指標及預后的關系。方法以帕金森伴慢性心衰患者46例為觀察組,另選取30例同期單純帕金森病患者為帕金森組,30例同期健康查體者為對照組。檢測比較3組入選次日的高頻(HF)、低頻(LF)和高頻、低頻比(HF/LF)等HRV指標和左室射血分數(shù)(LVEF)。統(tǒng)計觀察組隨訪半年的心血管事件發(fā)生率和死亡率,并分析帕金森伴慢性心衰患者HRV與其LVEF、心血管事件發(fā)生率和死亡率的關系。結果與帕金森組和對照組比較,觀察組的HF、HF/LF和LVEF均降低,LF則升高(P0.05)。與對照組比較,帕金森組HF、HF/LF和LVEF亦均降低,LF則升高(P0.05)。觀察組隨訪半年的心血管事件發(fā)生率和死亡率分別為17.39%和13.04%。觀察組LVEF≥50%患者的HF和HF/LF均高于LVEF50%患者,LF則低于LVEF50%患者(P0.05)。觀察組存活患者的HF和HF/LF均高于死亡患者,LF則低于死亡患者(P0.05)。且觀察組無心血管事件患者的HF和HF/LF均高于有心血管事件患者,LF則低于有心血管事件患者(P0.05)。Pearson線性相關分析結果顯示,帕金森伴慢性心衰患者HF和HF/LF與其LVEF均呈正相關(r=0.882、0.853,P0.05),其LF與其LVEF則呈負相關(r=-0.792,P0.05)。Spearman無條件相關分析結果顯示,帕金森伴慢性心衰患者HF和HF/LF與其心血管事件發(fā)生率和死亡率均呈負相關(HF:r=-0.812、-0.833;HF/LF:r=-0.825、-0.871,P0.05),其LF與其心血管事件發(fā)生率和死亡率則呈正相關(r=0.805、0.816,P0.05)。結論帕金森伴慢性心衰患者HRV降低且與其心功能降低和不良預后密切相關,HRV指標的檢測有助于其心功能和預后評估,指導其治療和預后干預。
[Abstract]:Objective to investigate the changes of heart rate variability (HRV) in patients with Parkinson's disease and chronic heart failure (CHF) and the relationship between HRV and cardiac function and prognosis. Methods 46 cases of Parkinson's disease with chronic heart failure as observation group and another 30 cases of Parkinson's disease as control group were selected as control group. The HRV index and left ventricular ejection fraction (LVEF) were measured and compared in three groups. The incidence and mortality of cardiovascular events and the relationship between HRV and LVEF, incidence of cardiovascular events and mortality in patients with Parkinson's disease and chronic heart failure were analyzed. Results compared with Parkinson's group and control group, both HFF / LF and LVEF in the observation group were decreased and LF was increased (P 0.05). Compared with the control group, HFR / LF and LVEF in PD group were decreased, and LF increased (P 0.05).
