口部感覺-運動評估表的編制及其在腦癱兒童中的應(yīng)用

【摘要】:口部感覺-運動障礙是導(dǎo)致腦癱兒童運動性言語障礙構(gòu)音不清的主要因素之一,但目前國內(nèi)缺乏針對腦癱兒童的口部-感覺運動的評估材料。本研究旨在編制口部感覺-運動評估材料,并將此評估材料應(yīng)用于3-7歲的腦癱兒童,從年齡、性別、粗大運動功能水平以及癱型四個方面對腦癱兒童的口部感覺-運動特征與正常兒童進行比較研究,探究腦癱兒童的口部感覺-運動特征。另外,本研究還驗證了腦癱兒童口部感覺-運動與構(gòu)音清晰度的相關(guān)性。本研究的主要內(nèi)容和結(jié)果包括以下幾個方面:1.口部感覺-運動評估表的編制本研究在盧紅云口部運動功能評估表的基礎(chǔ)上,增加口部感覺評估的內(nèi)容,補充口部運動的項目,并根據(jù)腦癱兒童言語障礙矯治臨床實踐的反饋對原評估表中某些項目的施測方法、分級標準進行調(diào)整。通過專家分析對該評估材料的項目進行篩選,最終形成口部感覺-運動評估表。對評估表的信效度分析表明評估表具有良好的信度、效度。同時本研究還獲得了 3-7歲普通兒童口部感覺-運動的年齡發(fā)育順序。2.口部感覺-運動評估表的應(yīng)用(1)普通和腦癱兒童口部感覺-運動的比較研究本研究通過分析普通和腦癱兒童口部感覺-運動評估結(jié)果發(fā)現(xiàn),年齡和性別對普通兒童的口部感覺-運動有顯著影響,但是對腦癱兒童的影響則不顯著。痙攣型腦癱兒童的口部感覺-運動極顯著差于普通兒童,其中以四肢癱患兒的口部感覺-運動障礙最為嚴重;痙攣型四肢癱患兒的下頜運動、唇運動、舌運動障礙均較為嚴重。粗大運動功能障礙越重,口部感覺-運動越差;中度與重度粗大運動功能障礙腦癱兒童口部感覺-運動障礙程度均較重,二者在感覺、下頜運動、唇運動、舌運動上均不存在顯著差異。(2)腦癱兒童口部感覺-運動與構(gòu)音清晰度的相關(guān)性研究本研究分別對腦癱兒童的口部感覺-運動與字清晰度和句清晰度的相關(guān)性進行分析,結(jié)果表明腦癱兒童的口部感覺-運動與字和句清晰度均存在中等程度相關(guān)。本研究編制了腦癱兒童口部感覺-運動評估表,得到了普通兒童口部感覺-運動的年齡發(fā)育順序,并探究了腦癱兒童的口部感覺-運動特征,還驗證了腦癱兒童口部感覺-運動與構(gòu)音清晰度的相關(guān)性,對腦癱兒童口部感覺-運動訓(xùn)練具有重要指導(dǎo)意義。
[Abstract]:Oral sensorimotor disorder is one of the main factors leading to poor articulation of motor speech disorder in children with cerebral palsy. The purpose of this study was to develop an oral sensory and motor assessment material and apply it to children with cerebral palsy aged 3-7 years, from age to sex. In order to explore the characteristics of oral sensorimotor motility in children with cerebral palsy, the level of coarse motor function and the four aspects of paralytic type were compared between children with cerebral palsy and normal children. In addition, this study also verified the correlation between sensory-motor and articulation in the mouth of children with cerebral palsy. The main contents and results of this study include the following aspects: 1. Preparation of the sensory and Motion Assessment Table for the mouth this study added the content of the sensory assessment of the mouth to supplement the items of the movement of the mouth on the basis of the evaluation table of the motor function of the mouth of Lu Hongyun. According to the feedback from the clinical practice of speech disorder correction in children with cerebral palsy, the test methods and grading criteria of some items in the original evaluation table were adjusted. The items of the evaluation material were screened by expert analysis, and the sensory-motion assessment table was formed. The reliability and validity analysis of the evaluation table shows that the evaluation table has good reliability and validity. At the same time, the age developmental sequence of sensory-motor in the mouth of 3-7 years old children was obtained. A comparative study of the oral sensorimotor between common children and children with cerebral palsy by analyzing the results of oral sensorimotor assessment in common children and children with cerebral palsy. Age and gender had significant effects on oral sensorimotor in normal children, but not on children with cerebral palsy. The oral sensorimotor dysfunction of spastic cerebral palsy children was worse than that of the common children, especially in the quadriplegic children, and the mandibular movement, lip movement and tongue motility of the spastic quadriplegia children were more serious. The more severe the gross motor dysfunction, the worse the sensation and movement of the mouth, the more severe the sensory and motor disturbance in the mouth of children with moderate and severe motor dysfunction, the more severe the sensation, the movement of the mandible, the movement of the lips, the more severe the degree of the sensory and motor disorders in the mouth. There was no significant difference in tongue movement. (2) the correlation between oral sensorimotor and articulation in children with cerebral palsy was studied. This study analyzed the correlation between mouth-sensorimotor and word clarity and sentence articulation in children with cerebral palsy. The results showed that there was a moderate correlation between oral sensorimotor and articulation of words and sentences in children with cerebral palsy. In this study, a sensory motor evaluation table was compiled for children with cerebral palsy, and the age developmental sequence of oral sensorimotor in common children was obtained, and the characteristics of oral sensorimotor in children with cerebral palsy were investigated. In addition, the correlation between oral sensorimotor and articulation in children with cerebral palsy was verified, which is of great significance to the training of sensory motor in mouth of children with cerebral palsy.
