【摘要】：目的： 探討不同病因的周圍性面癱患者相關(guān)臨床診療特點。 方法： 收集自2008年1月至2013年12月期間福建醫(yī)科大學(xué)附屬第一醫(yī)院耳鼻咽喉科收治住院的資料完整的面神經(jīng)麻痹患者100例，針對不同病因分別進(jìn)行保守治療或手術(shù)治療，對各組面癱的臨床診療情況進(jìn)行分析。 結(jié)果： 周圍性面癱發(fā)病各病因所占百分比：中耳炎37％，腫瘤13%，貝爾面癱12％，Hunt綜合癥為12%，，外傷性為8%，醫(yī)源性為6%，先天性為5%，既往面癱為7%。根據(jù)不同病程及不同程度的周圍性面癱將患者進(jìn)行分析，發(fā)現(xiàn)病程越短，面癱恢復(fù)越好；面神經(jīng)功能分級越低，面癱恢復(fù)越好。 結(jié)論： 周圍性面癱的預(yù)后與病程及面癱程度有關(guān)。針對不同病因?qū)е碌闹車悦姘c，均應(yīng)進(jìn)行定性及定位診斷，經(jīng)過嚴(yán)格的面神經(jīng)電生理診斷以及影像學(xué)檢查，確定合適的治療方案。經(jīng)保守治療無效，應(yīng)盡早選擇手術(shù)治療。
[Abstract]:Objective: To investigate the clinical features of peripheral facial palsy with different etiology. Methods: From January 2008 to December 2013, 100 patients with facial nerve paralysis admitted to the Department of Otorhinolaryngology, the first affiliated Hospital of Fujian Medical University were collected. The clinical diagnosis and treatment of facial paralysis in each group were analyzed. Results: The percentage of etiological factors of peripheral facial paralysis was: otitis media 37, tumor 13, Bell's facial palsy 12, traumatic 8, iatrogenic 6, congenital 5 and previous facial paralysis 7. According to the different course of disease and different degrees of peripheral facial paralysis, we found that the shorter the course of disease, the better the recovery of facial paralysis, the lower the grade of facial nerve function, the better the recovery of facial paralysis. Conclusion: The prognosis of peripheral facial paralysis is related to the course of disease and the degree of facial paralysis. For peripheral facial paralysis caused by different etiology, qualitative and localization diagnosis should be carried out. After strict electrophysiological diagnosis of facial nerve and imaging examination, the appropriate treatment plan should be determined. If conservative treatment is ineffective, surgical treatment should be chosen as soon as possible.
