【摘要】：正脊髓電刺激療法(spinal cord stimulation,SCS),俗稱(chēng)鎮(zhèn)痛起搏器。是指將刺激電極置入椎管的硬膜外腔,通過(guò)電流刺激脊髓后角感覺(jué)神經(jīng)元及后柱傳導(dǎo)束,從而阻斷疼痛信號(hào)傳導(dǎo),以達(dá)到治療疼痛的一種神經(jīng)調(diào)控方法。SCS早已被美國(guó)FDA批準(zhǔn)成為治療軀干和四肢慢性疼痛的一種方法。SCS發(fā)展了近40年,針對(duì)慢性疼痛的治療,其可行性及高選擇性有目共睹。然而,脊髓電刺激療法仍然面臨著其特有的一系列挑戰(zhàn),本文將就椎管內(nèi)治療策略、電刺激參數(shù)的選擇、患者的選擇、治療的成本效益以及提高療效的策略等問(wèn)題進(jìn)行詳細(xì)描述。
[Abstract]:Spinal cord stimulation, commonly known as analgesic pacemaker. It means that the stimulation electrode is placed into the epidural cavity of the spinal canal, and the pain signal transduction is blocked by electric stimulation of the sensory neurons and the posterior column conduction bundle of the posterior horn of the spinal cord. SCS has been approved by the United States FDA for the treatment of chronic pain in the trunk and extremities for nearly 40 years. The feasibility and high selectivity of the treatment for chronic pain is obvious to all. However, spinal cord electrical stimulation still faces a series of unique challenges. In this paper, we will discuss the treatment strategy of spinal canal, the choice of electrical stimulation parameters, the choice of patients. The cost-effectiveness of treatment and strategies to improve efficacy are described in detail.
