【摘要】：目的評價頸動脈超聲聯(lián)合同型半胱氨酸、糖化血紅蛋白以及血脂水平在診斷腦梗死中的價值。方法腦梗死患者100例(實驗組),按照病史及血糖檢測繼續(xù)分為糖尿病組、高血糖組以及正常血糖組,同期體檢健康人員60例(對照組),頸動脈彩超檢測實驗組及對照組人員頸動脈內(nèi)中膜厚度以及全自動生化分析儀檢測外周血中同型半胱氨酸、糖化血紅蛋白、血脂相關(guān)指標水平。結(jié)果實驗組檢測出70例頸動脈不同程度的內(nèi)中膜增厚,發(fā)生率為70%(χ2=54.1,P0.001);實驗組的HCY,HbA1c,TC,TG,LDL均高于正常對照組,而HDL低于正常對照組(t=6.25,10.6,13.6,6.3,5.39,-4.2;P均0.005);實驗組中頸動脈內(nèi)中膜厚度明顯增高(t=12.3,P0.001);糖尿病組與高血糖組的糖化血紅蛋白均高于正常血糖組(t=-13.4~-10.7,P均0.005),并且糖尿病組與高血糖組頸動脈彩超顯示頸動脈內(nèi)中膜厚度要明顯高于正常血糖組(t=-8.98~-2.88,P均0.005)。結(jié)論腦梗死的發(fā)病率與頸動脈粥樣硬化密切相關(guān);糖化血紅蛋白水平與腦梗死的病情程度密切相關(guān),可以作為判斷病情的有效指標;頸動脈彩超聯(lián)合腦卒中相關(guān)指標檢測可以提高腦梗死的檢出率。
[Abstract]:Objective to evaluate the value of carotid ultrasound combined with homocysteine, glycosylated hemoglobin and serum lipids in the diagnosis of cerebral infarction. Methods 100 patients with cerebral infarction (experimental group) were divided into diabetic group, hyperglycemia group and normal blood glucose group according to their history and blood glucose test. In the same period, 60 healthy persons (control group, carotid artery color Doppler ultrasound, carotid artery intima media thickness and automatic biochemical analyzer were used to detect homocysteine and glycosylated hemoglobin in peripheral blood. Serum lipids related index level. Results 70 cases of carotid artery thickening were detected in the experimental group, the incidence was 70 (蠂 2 / 54.1 / P 0.001), and the levels of HCYN HbA1 / TCU TGG / LDL in the experimental group were significantly higher than those in the control group (P < 0. 05). HDL was significantly lower than that in the normal control group (P 0.005), the thickness of carotid intima was significantly higher in the experimental group than that in the control group, and the glycosylated hemoglobin levels in the diabetic group and hyperglycemia group were higher than those in the normal blood glucose group (t -13.4- 10.7g P 0.005), and the carotid artery thickness in the diabetic group and the hyperglycemia group was significantly higher than that in the diabetic group and the hyperglycemia group (P = 0.005), while in the diabetic group and the hyperglycemia group, the level of glycosylated hemoglobin was significantly higher than that in the control group. The intima media thickness of carotid artery was significantly higher than that of normal blood glucose group (P 0.005). Conclusion the incidence of cerebral infarction is closely related to carotid atherosclerosis, and the level of glycosylated hemoglobin is closely related to the severity of cerebral infarction. Carotid ultrasound combined with stroke related indexes can improve the detection rate of cerebral infarction.
