腦梗塞復(fù)發(fā)的預(yù)測因素探討

【摘要】:目的:腦梗塞復(fù)發(fā)率高,影響復(fù)發(fā)因素多,為了探討腦梗塞復(fù)發(fā)可能的預(yù)測因素,我們選擇高敏C-反應(yīng)蛋白、內(nèi)皮素-1、纖維蛋白原等常見因素,對(duì)腦梗塞進(jìn)行為期二年的隨訪,以了解這幾個(gè)因素預(yù)測腦梗塞復(fù)發(fā)的可能。 方法:選擇在福建醫(yī)科大學(xué)附屬第二醫(yī)院2011.09~2012.03住院的首次腦梗塞患者123例,在入院24小時(shí)內(nèi)檢查高敏C-反應(yīng)蛋白、內(nèi)皮素-1、纖維蛋白原,并對(duì)每位患者進(jìn)行門診隨訪2年,患者死亡或新的腦梗塞發(fā)生時(shí),終止隨訪。 結(jié)果:經(jīng)過2年的隨訪,,其中31例在2年隨訪內(nèi)死亡或新的腦梗塞發(fā)生,復(fù)發(fā)率為25.2%。對(duì)31例復(fù)發(fā)的腦梗塞患者不同濃度高敏C-反應(yīng)蛋白、內(nèi)皮素-1、纖維蛋白原進(jìn)行分層后,分別各自算出其復(fù)發(fā)率,P值均小于0.05,有顯著性意義。再經(jīng)過多元logistic回歸分析表明高濃度高敏C-反應(yīng)蛋白、內(nèi)皮素-1、纖維蛋白原與患者死亡或新的腦梗塞發(fā)生有關(guān)。 結(jié)論:首次腦梗塞患者血清高敏C-反應(yīng)蛋白、內(nèi)皮素-1、纖維蛋白原濃度與腦梗塞復(fù)發(fā)有顯著相關(guān)性,是首次腦梗塞患者復(fù)發(fā)的獨(dú)立危險(xiǎn)因素。
[Abstract]:Objective: the recurrence rate of cerebral infarction is high, and there are many factors affecting the recurrence. In order to explore the possible predictors of cerebral infarction recurrence, we chose Gao Min C-reactive protein, endothelin-1, fibrinogen and other common factors to follow up cerebral infarction for two years. To understand these factors to predict the recurrence of cerebral infarction. Methods: 123 patients with first cerebral infarction were selected from the second affiliated Hospital of Fujian Medical University. Gao Min C-reactive protein, endothelin-1 and fibrinogen were examined within 24 hours of admission, and each patient was followed up for 2 years. The follow-up was terminated when the patient died or a new cerebral infarction occurred. Results: after 2 years follow up, 31 cases died or new cerebral infarction occurred during 2 years follow-up, the recurrence rate was 25. 2%. After stratification of Gao Min C-reactive protein, endothelin-1 and fibrinogen in 31 patients with recurrent cerebral infarction, the recurrence rate (P < 0.05) was calculated respectively. Multiple logistic regression analysis showed that high concentrations of Gao Min C-reactive protein, endothelin -1 and fibrinogen were associated with death or new cerebral infarction. Conclusion: the serum concentration of Gao Min C-reactive protein, endothelin -1 and fibrinogen in patients with first cerebral infarction is significantly correlated with the recurrence of cerebral infarction, and is an independent risk factor for recurrence in patients with first cerebral infarction.
