【摘要】：目的探討血清脂蛋白相關(guān)磷脂酶A2(Lp-PLA2)、B型腦鈉肽(BNP)在急性缺血性腦卒中早期診斷中的價(jià)值。方法選擇急性缺血性腦卒中患者112例為病例組,57例健康者為對(duì)照組,測(cè)定血清Lp-PLA2、缺血修飾蛋白(IMA)、TC、TG、LDL-C、HDL-C、脂蛋白a[Lp(a)]及血漿D-二聚體(D-D)、BNP水平并進(jìn)行統(tǒng)計(jì)分析。結(jié)果病例組Lp-PLA2、BNP、IMA、LDL-C、Lp(a)、D-D水平高于對(duì)照組,HDL-C水平低于對(duì)照組(P0.05);兩組TC、TG水平差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。多因素Logistic回歸分析顯示,吸煙、糖尿病、Lp-PLA2、BNP、IMA、HDL-C為急性缺血性腦卒中的獨(dú)立影響因素(P0.05)。Spearman秩相關(guān)分析顯示,血清Lp-PLA2水平與LDL-C、Lp(a)、IMA、D-D水平和頸總動(dòng)脈內(nèi)中膜厚度(IMT)呈正相關(guān)(r=0.185、0.227、0.270、0.364和0.225,P0.05),與HDL-C呈負(fù)相關(guān)(r=-0.280,P0.01);血漿BNP水平與IMA和IMT呈正相關(guān)(r=0.201和0.217,P0.05)。結(jié)論 Lp-PLA2和BNP參與急性缺血性腦卒中的病理生理過(guò)程。
[Abstract]:Objective to investigate the value of serum lipoprotein associated phospholipase A 2 (Lp-PLA 2) B type brain natriuretic peptide (BNP) in the early diagnosis of acute ischemic stroke.Methods A total of 112 patients with acute ischemic stroke were selected as control group and 57 healthy controls. The levels of serum Lp-PLA2, Ischemia-modified protein TGG, LDL-C, lipoprotein a and plasma D-dimer D-DU BNP were measured and analyzed statistically.Results the level of Lp-PLA2BNPAIMA LDL-CU D-D was higher than that of the control group (P 0.05), but there was no significant difference between the two groups in the level of TCU TG (P0.05P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05).Multivariate Logistic regression analysis showed that smoking and diabetes mellitus (Lp-PLA2) BNP-IMAL-C were independent influencing factors of acute ischemic stroke (P 0.05n. Spearman rank correlation analysis).The level of serum Lp-PLA2 was positively correlated with the level of IMA D-D and IMT of the common carotid artery, and negatively correlated with HDL-C. The level of plasma BNP was positively correlated with IMA and IMT.Conclusion Lp-PLA2 and BNP are involved in the pathophysiological process of acute ischemic stroke.
