基于ICF-CY的腦性癱瘓上肢功能評估量表內(nèi)容效度分析

【摘要】:目的以《國際功能、殘疾和健康分類(兒童及青少年版)》(ICF-CY)為框架,明確三種常用腦癱兒童上肢功能評估量表的內(nèi)容和臨床使用價(jià)值。方法選擇腦癱兒童精細(xì)運(yùn)動功能測試量表(FMFM)、墨爾本單側(cè)上肢功能評估量表2(MA2)和上肢技巧質(zhì)量測試量表(QUEST),由2名從事腦癱兒童上肢功能評估3年以上的康復(fù)醫(yī)生和治療師獨(dú)立確定每個(gè)條目所包含的概念,按相關(guān)聯(lián)系法則將提取的概念I(lǐng)CF-CY類目建立關(guān)聯(lián),分析各量表提取的概念數(shù)、涉及ICF-CY類目數(shù)以及與之發(fā)生關(guān)聯(lián)數(shù)的分布狀況。結(jié)果三個(gè)量表共計(jì)提取62個(gè)概念,發(fā)生403次關(guān)聯(lián)。FMFM關(guān)聯(lián)的ICF-CY類目最多,"活動和參與"類目占關(guān)聯(lián)總數(shù)近2/3(134/204)。QUEST主要與身體功能的下級類目"關(guān)節(jié)活動和手臂的支撐功能"發(fā)生關(guān)聯(lián)。MA2與反映運(yùn)動準(zhǔn)確性的知覺功能類目關(guān)聯(lián)7次,涉及運(yùn)動流暢性的類目關(guān)聯(lián)9次,合計(jì)占總關(guān)聯(lián)數(shù)50%以上。結(jié)論 FMFM更多地評價(jià)腦癱兒童上肢功能的活動和參與能力,也更為適合年幼腦癱兒童;MA2更多地關(guān)注上肢運(yùn)動的準(zhǔn)確度和流暢性;而QUEST主要關(guān)注身體功能,尤其是上肢的關(guān)節(jié)活動和支撐功能。
[Abstract]:Objective to determine the content and clinical value of three common upper limb function assessment scales for children with cerebral palsy (ICF-CYL), using the International Classification of function, Disability and Health (Child & Adolescent version) as the framework.Methods the fine motor function test scale for children with cerebral palsy (FMFMN), the Melbourne upper limb function assessment scale (2MA2) and the upper limb skill quality scale (questt) were selected. Two rehabilitation doctors who were engaged in the evaluation of upper limb function of children with cerebral palsy for more than 3 years were selected.The therapist independently identifies the concepts contained in each entry,According to the correlation relation rule, the concepts of ICF-CY category are associated, and the number of concepts extracted from each scale is analyzed, which involves the number of ICF-CY categories and the distribution of the associated numbers.Results A total of 62 concepts were extracted from the three scales."activity and participation" category accounted for the total number of associations. The "activity and participation" category was mainly associated with the lower category of physical function, "Joint activity and arm support function". MA2 was associated with the accuracy of reflecting movement.The perceptual functional categories were associated with each other 7 times,There were 9 associations involving motion fluency, which accounted for more than 50% of the total correlation.Conclusion FMFM is more suitable for evaluating the upper limb function of children with cerebral palsy, and is more suitable for young children with cerebral palsy to pay more attention to the accuracy and fluency of upper limb movement, while QUEST is mainly concerned with physical function.In particular, the upper limb joint movement and support function.
