【摘要】：目的探討早期實(shí)施高壓氧聯(lián)合康復(fù)治療對老年高血壓腦出血(HICH)患者神經(jīng)功能恢復(fù)及預(yù)后的影響。方法老年HICH患者300例,隨機(jī)數(shù)字表法分為研究組和治療組各150例,研究組采用常規(guī)治療聯(lián)合早期高壓氧和康復(fù)治療,治療組采用常規(guī)治療+早期高壓氧方法,采用美國國立衛(wèi)生研究院卒中量表(NIHSS)、歐洲卒中評分量表(ESS)、簡易智能精神狀態(tài)檢查量表(MMSE)評估兩組神經(jīng)及運(yùn)動功能變化,采用格拉斯哥預(yù)后評分(GOS)標(biāo)準(zhǔn)評估治療情況。采用中文版Nasreddine檢查表(MOCA)評價患者的認(rèn)知功能,按照Fugl-Meyer運(yùn)動功能評分評估患者的肢體運(yùn)動情況,采用日常生活活動能力(ADL)量表評價兩組日�；顒幽芰�。對比分析治療30 d后神經(jīng)功能恢復(fù)情況,治療6個月后預(yù)后情況。結(jié)果治療30 d后,研究組NIHSS、ESS評分均低于治療組,MMSE評分高于治療組(P0.01);治療6個月后,研究組認(rèn)知功能、運(yùn)動功能、日常生活活動能力評分均明顯高于治療組,GOS評分顯示研究組預(yù)后良好率明顯高于治療組(均P0.01)。結(jié)論早期高壓氧聯(lián)合康復(fù)治療具有緩解老年HICH患者昏迷情況,促進(jìn)患者神經(jīng)及日常運(yùn)動功能恢復(fù),改善預(yù)后、提高生存質(zhì)量。
[Abstract]:Objective to investigate the effect of early hyperbaric oxygen combined rehabilitation therapy on neurological function recovery and prognosis in elderly patients with hypertensive intracerebral hemorrhage (HICH).Methods 300 elderly patients with HICH were randomly divided into study group (n = 150) and treatment group (n = 150). The study group was treated with routine therapy combined with early hyperbaric oxygen and rehabilitation, and the treatment group was treated with routine treatment of early hyperbaric oxygen.The changes of neurologic and motor function were evaluated by NIHSS, ESS and MMSE, respectively. Glasgow prognosis scale (GOS) was used to evaluate the treatment status of the two groups.The cognitive function of the patients was evaluated by the Chinese version of the Nasreddine scale, the limb movement of the patients was evaluated according to the Fugl-Meyer motor function score, and the ADL scale was used to evaluate the daily activity ability of the two groups.The recovery of nerve function after 30 days treatment and the prognosis after 6 months treatment were compared and analyzed.Results after 30 days of treatment, the scores of NIHSS ESS in the study group were lower than those in the treatment group, and the scores of MMSE in the study group were higher than those in the treatment group (P 0.01), and after 6 months of treatment, the cognitive function and motor function were higher in the study group.The scores of ADL were significantly higher than that of GOS in the treatment group (P < 0.01), and the prognostic rate of the study group was significantly higher than that of the treatment group (P < 0.01).Conclusion early hyperbaric oxygen combined rehabilitation therapy can relieve the coma in elderly patients with HICH, promote the recovery of nerve and daily motor function, improve the prognosis and improve the quality of life.
