【摘要】：目的：探討血清MMP-9水平與腦梗死的關(guān)系，分析其與腦梗死病因的相關(guān)性，了解血清MMP-9水平的檢測(cè)對(duì)評(píng)估腦梗死患者的病情及預(yù)后的臨床意義。 方法：選擇發(fā)病后48小時(shí)內(nèi)的急性腦梗死患者80例作為腦梗死組（CI組），經(jīng)頭顱CT或MRI檢查確診，隨機(jī)分為TOAST分型中的大動(dòng)脈粥樣硬化性梗死組40例（LAA組），心源性腦栓塞組40例（CCE組），，另選擇40例入院體檢者作為對(duì)照組（C組）。于入院次日清晨空腹抽取靜脈血，采用酶聯(lián)免疫吸附（ELISA）法測(cè)定血清MMP-9水平。結(jié)果用SPSS17.0軟件包進(jìn)行統(tǒng)計(jì)學(xué)分析。 結(jié)果：1.CI組患者血清MMP-9水平高于C組，兩者差異具有統(tǒng)計(jì)學(xué)意義（P0.01）；2.LAA組患者血清MMP-9水平與CCE組差異無(wú)明顯統(tǒng)計(jì)學(xué)意義（P0.05）；3.將CI組患者進(jìn)行美國(guó)國(guó)立衛(wèi)生院卒中量表（NIHSS）評(píng)分發(fā)現(xiàn)，血清MMP-9水平與NIHSS評(píng)分呈正相關(guān)（相關(guān)系數(shù)r=0.583，P0.01）；4.根據(jù)NIHSS評(píng)分分組比較血清MMP-9水平，差異有統(tǒng)計(jì)學(xué)意義（P0.01）；5.Logistic回歸分析發(fā)現(xiàn)，MMP-9與腦梗死的多種因素具有相關(guān)性（回歸系數(shù)β=1.513，P0.01）。 結(jié)論：1.腦梗死組患者血清MMP-9水平較對(duì)照組明顯升高，提示MMP-9可能參與了腦梗死的發(fā)病過(guò)程。2.大動(dòng)脈粥樣硬化性腦梗死與心源性腦栓塞患者血清MMP-9水平變化無(wú)明顯差異。3.腦梗死患者血清MMP-9水平與神經(jīng)功能缺損程度呈正相關(guān)，血清MMP-9水平可提示腦梗死患者的病情及預(yù)后。
[Abstract]:Objective: to investigate the relationship between serum MMP-9 level and cerebral infarction, to analyze the relationship between serum MMP-9 level and the etiology of cerebral infarction, and to understand the clinical significance of serum MMP-9 level in evaluating the condition and prognosis of patients with cerebral infarction.Methods: 80 patients with acute cerebral infarction within 48 hours after onset were selected as CI group and diagnosed by CT or MRI.The patients were randomly divided into TOAST classification group (n = 40), Atherosclerotic infarction group (n = 40), cardiogenic cerebral embolism group (n = 40) and control group C (n = 40).Venous blood was collected on an empty stomach the next day after admission and serum MMP-9 level was determined by Elisa.Results SPSS17.0 software package was used for statistical analysis.Results 1. The serum MMP-9 level in CI group was higher than that in C group, and the difference was statistically significant. 2. There was no significant difference in serum MMP-9 level between LAA group and CCE group.The stroke scale (NIHSS) score of CI group showed that the serum MMP-9 level was positively correlated with the NIHSS score (r = 0. 583, P 0. 01, P 0. 01).The serum MMP-9 levels were compared according to the NIHSS scores. 5. Logistic regression analysis showed that there was a correlation between MMP-9 and many factors of cerebral infarction (regression coefficient 尾 1. 513 + P 0. 01).Conclusion 1.The level of serum MMP-9 in cerebral infarction group was significantly higher than that in control group, suggesting that MMP-9 might be involved in the pathogenesis of cerebral infarction.There was no significant difference in serum MMP-9 levels between patients with atherosclerotic cerebral infarction and cardiogenic cerebral embolism.The level of serum MMP-9 was positively correlated with the degree of neurological impairment in patients with cerebral infarction, and the level of serum MMP-9 could indicate the condition and prognosis of patients with cerebral infarction.
