運動神經(jīng)元病患者腦白質(zhì)病變的擴散張量成像研究

【摘要】:目的利用擴散張量成像技術(shù)(DTI)定量評估運動神經(jīng)元病患者的腦白質(zhì)病變,探究可能的發(fā)病機制。方法15例臨床診斷為運動神經(jīng)元病患者作為實驗組,16例年齡、性別匹配的無神經(jīng)系統(tǒng)疾病的志愿者為對照組。行DTI及常規(guī)磁共振掃描,應(yīng)用基于纖維束的空間統(tǒng)計方法(TBSS)對全腦白質(zhì)進行分析,提取異常區(qū)域的部分各項異性(FA值)、平均彌散率(MD值)、特征值(λ1、λ2、λ3)。結(jié)果實驗組與對照組相比,左側(cè)皮質(zhì)脊髓束FA值顯著下降(P=0.034),特征值λ2(P=0.124)、λ3(P=0.064)有升高趨勢,右側(cè)皮質(zhì)脊髓束FA值有下降趨勢(P=0.050)。兩組λ1、MD值未見明顯差異。結(jié)論 DTI能夠定量顯示運動神經(jīng)元病患者皮質(zhì)脊髓束損害,并且該病變可能是脫髓鞘所致。
[Abstract]:Objective using diffusion tensor imaging (DTI) quantitative assessment of cerebral white matter lesions in patients with motor neuron disease, to explore the possible pathogenesis. Methods 15 patients with motor neuron disease patients as the experimental group, 16 cases of age, gender, without neurological diseases volunteers as control group underwent DTI and conventional. Magnetic resonance imaging, the application of statistical method based on fiber bundle space (TBSS) on cerebral white matter analysis, extraction of anisotropic abnormal area part (FA), average diffusion rate (MD value), eigenvalue (lambda 1, lambda 2, lambda 3). The results of the experimental group compared with the control group on the left side of the corticospinal tract FA value decreased significantly (P=0.034), (P=0.124) eigenvalue lambda 2, lambda 3 (P=0.064) increased, on the right side of the corticospinal tract FA value decreased (P=0.050). Two groups of lambda 1, there was no significant difference between the values of MD. Conclusion DTI can quantitatively show the corticospinal motor neuron disease beam Damage, and the lesion may be caused by demyelination.
