【摘要】：目的:觀察活血化瘀解郁方治療氣滯血瘀型腦卒中后抑郁(Post-stroke depression,PSD)患者的臨床療效。方法:選取60例氣滯血瘀型腦卒中后抑郁患者作為研究對象,按就診順序隨機(jī)分配治療組與對照組。中藥治療組及對照組各30例,部分入組病例在研究期間出現(xiàn)脫落,最終治療組27例、對照組26例。治療組予活血化瘀解郁方加口服草酸艾司西酞普蘭片(來士普)治療(中藥每日一貼,分早、晚二次溫水沖服,草酸艾司西酞普蘭片,1Omg 1/早);對照組口服草酸艾司西酞普蘭片,1Omg 1/早治療。兩組患者均給予神經(jīng)內(nèi)科腦血管病常規(guī)治療。兩組療程均6周。于治療前、治療2周、4周及6周時(shí),分別對兩組患者進(jìn)行漢密爾頓抑郁量表(HAMD)評分,美國國立衛(wèi)生研究院神經(jīng)功能缺損量表(NIHSS)評分、Barthel指數(shù)評分及中醫(yī)癥候評分。治療組與對照組比較,同時(shí)在臨床實(shí)驗(yàn)期間進(jìn)行常規(guī)安全性檢查,評價(jià)活血化瘀解郁方治療氣滯血瘀型腦卒中后抑郁的臨床治療效果及用藥安全性。結(jié)果:治療組在降低漢密爾頓抑郁量表(HAMD)評分,神經(jīng)功能缺損量表(NIHSS)評分及中醫(yī)癥候評分,提高Barthel指數(shù)評分均明顯優(yōu)于對照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。對不良反應(yīng)的觀察,兩組患者均未出現(xiàn)嚴(yán)重不良反應(yīng),提示本方運(yùn)用于臨床治療腦卒中后抑郁是安全的。結(jié)論:活血化瘀解郁方治療氣滯血瘀型腦卒中后抑郁,療效確切,不僅能顯著改善患者抑郁癥狀,并且可以促進(jìn)患者神經(jīng)功能缺損的恢復(fù),提高患者獨(dú)立生活能力,且無不良反應(yīng),安全有效。
[Abstract]:Objective: to observe the clinical effect of Huoxue Huayu Jieyu recipe in treating post stroke depression of qi stagnation and blood stasis type. Methods: 60 patients with post stroke depression of qi stagnation and blood stasis type were selected as study objects. The treatment group and the control group were randomly assigned according to the order of visit. 30 cases each in the traditional Chinese medicine treatment group and 30 cases in the control group, some of the cases in the group fell off during the study period, and 27 cases in the final treatment group, In the control group, 26 cases were treated with Huoxue Huayu Jieyu decoction plus oral estalopram oxalate (Lestopram). Oxalcitalopram oxalate (1Omg / L) and control group (control group) were treated orally with oxalcitalopram (1Omg / L). The patients in both groups were given routine treatment of cerebrovascular diseases in neurology department. The course of treatment in both groups was 6 weeks. Before and after treatment, the patients were treated for 2 weeks, 4 weeks and 6 weeks, respectively. Two groups of patients were evaluated with Hamilton Depression scale (Hamd), National Institutes of Health (NIH) Neurologic deficit scale (NIHSS), Barthel Index (BI) and TCM symptom scale (TCM). At the same time, routine safety examination was carried out during the clinical trial to evaluate the efficacy and safety of Huoxue Huayu Jieyu recipe in the treatment of post-stroke depression of Qi stagnation and Blood stasis type. Results: the treatment group was reducing the score of Hamilton Depression scale (Hamd). The scores of neurological deficit scale (NDS) and TCM symptoms, and the scores of improving Barthel index were significantly better than those of the control group (P 0.05). No serious adverse reactions were found in the two groups. Conclusion: Huoxue Huayu Jieyu prescription is effective in treating post-stroke depression of Qi stagnation and Blood stasis type. It can promote the recovery of neurological deficit, improve the independent living ability of patients, no adverse reactions, safe and effective.
