【摘要】：目的探討以突聾為首發(fā)癥狀的腦梗死患者的臨床特點(diǎn),提高對(duì)該類情況的認(rèn)識(shí),盡早做出正確診斷。方法報(bào)道我院2014年收治的2例以突聾為首發(fā)癥狀的腦梗死病例,均完善頭顱MRI、HR-MRI、DSA、頸動(dòng)脈超聲、TCD檢查,結(jié)合文獻(xiàn)復(fù)習(xí),總結(jié)臨床特點(diǎn)。結(jié)果該2例患者首發(fā)癥狀表現(xiàn)為突聾合并頭暈,DSA均發(fā)現(xiàn)基底動(dòng)脈近段狹窄。病例1頭顱MRI示雙側(cè)橋腦及左側(cè)小腦新發(fā)梗死灶,予急診動(dòng)脈溶栓治療,后續(xù)雙聯(lián)抗血小板聚集及他汀治療,聽(tīng)力好轉(zhuǎn)。病例2頭顱MRI示右側(cè)橋腦新發(fā)梗死灶,予雙聯(lián)抗血小板聚集及他汀治療,遺留聽(tīng)力下降。結(jié)論以突聾為首發(fā)癥狀的腦梗死并不罕見(jiàn),突聾患者應(yīng)盡快完善腦血管檢查,突聾患者具有以下臨床特點(diǎn)時(shí)需要高度警惕腦梗死:伴隨頭暈癥狀、合并多種動(dòng)脈粥樣硬化危險(xiǎn)因素、基底動(dòng)脈狹窄。
[Abstract]:Objective to investigate the clinical characteristics of patients with cerebral infarction with sudden deafness as the first symptom, to improve the understanding of this condition and to make a correct diagnosis as soon as possible. Methods two cases of cerebral infarction with sudden deafness as the first symptom were reported in our hospital on 2014. HR-MRIV DSAs of head MRI, TCD examination of carotid artery ultrasound, and review of literature, Results the first symptom of the two patients was sudden deafness combined with dizziness and stenosis of proximal basilar artery. In case 1, cranial MRI showed bilateral pontine and left cerebellar infarction, and was treated with emergency arterial thrombolytic therapy. In case 2, MRI showed a new infarct in the right pontine, which was treated with dual antiplatelet aggregation and statins. Conclusion Cerebral infarction with sudden deafness as the first symptom is not rare. Cerebral vascular examination should be improved as soon as possible in patients with sudden deafness. Patients with sudden deafness should be on high alert for cerebral infarction when they have the following clinical characteristics: accompanied by dizziness symptoms. Associated with multiple risk factors for atherosclerosis, basilar artery stenosis.
