【摘要】:背景 頭痛在困擾人類(lèi)的疼痛中是發(fā)病頻率最高的,也是臨床最常見(jiàn)的癥狀之一。原發(fā)性頭痛以偏頭痛、緊張型頭痛、叢集性頭痛為多見(jiàn),繼發(fā)性頭痛中藥物過(guò)量性頭痛是一類(lèi)特殊的、常見(jiàn)的頭痛類(lèi)型。目前尚無(wú)簡(jiǎn)單可行、準(zhǔn)確度高的生物學(xué)指標(biāo)來(lái)明確各種頭痛的類(lèi)型。本課題擬通過(guò)對(duì)頭痛患者和健康人群外周血單個(gè)核細(xì)胞中PACAP、BDNF的表達(dá)進(jìn)行檢測(cè),探討PACAP、BDNF與頭痛人群的相關(guān)性,為臨床頭痛診斷及鑒別診斷尋找簡(jiǎn)便可行、準(zhǔn)確度高的生物學(xué)指標(biāo),為頭痛的機(jī)制研究提供新的理論依據(jù)。全文共包括兩部分內(nèi)容:第一部分PACAP在頭痛患者外周血單個(gè)核細(xì)胞中mRNA表達(dá)目的研究頭痛人群外周血單個(gè)核細(xì)胞中PACAP mRNA表達(dá)水平方法 選擇2014年04月~2015年02月間解放軍總醫(yī)院頭痛門(mén)診的頭痛患者101例,根據(jù)I CHD-3 beta版診斷標(biāo)準(zhǔn)分為4組:偏頭痛組、緊張型頭痛組、叢集性頭痛組及藥物過(guò)量性頭痛組;另選健康人35例作為正常對(duì)照組。分離外周血單個(gè)核細(xì)胞,提取總RNA,逆轉(zhuǎn)錄合成cDNA,采用Real-time PCR檢測(cè),以得到的PACAP基因Ct值與GAPDH基因Ct值的差值作為外周血單個(gè)核細(xì)胞中PACAP mRNA相對(duì)表達(dá)含量,對(duì)頭痛組各組與正常對(duì)照組進(jìn)行比較。結(jié)果偏頭痛組與正常對(duì)照組比較外周血單個(gè)核細(xì)胞中的PACAP mRNA表達(dá)水平降低,差異有統(tǒng)計(jì)學(xué)意義(P0.05);正常對(duì)照組與緊張型頭痛組、叢集性頭痛組、藥物過(guò)量性頭痛組比較外周血單個(gè)核細(xì)胞中的PACAP mRNA表達(dá)水平無(wú)差異表達(dá)。結(jié)論外周血單個(gè)核細(xì)胞中的PACAP mRNA表達(dá)水平與偏頭痛密切相關(guān),與緊張型頭痛、叢集性頭痛、藥物過(guò)量性頭痛無(wú)明顯相關(guān)。有助于偏頭痛與緊張型頭痛、叢集性頭痛和藥物過(guò)量性頭痛這幾種常見(jiàn)頭痛類(lèi)型鑒別,并為以后的機(jī)制研究提供新的理論依據(jù)。第二部分BDNF在頭痛患者外周血單個(gè)核細(xì)胞中mRNA表達(dá)目的研究頭痛人群外周血單個(gè)核細(xì)胞中BDNF mRNA表達(dá)水平方法 選擇2014年04月~2015年02月間解放軍總醫(yī)院頭痛門(mén)診頭痛患者101例,根據(jù)ICHD-3 beta版診斷標(biāo)準(zhǔn)分為4組:偏頭痛組、緊張型頭痛組、叢集性頭痛組及藥物過(guò)量性頭痛組;另選健康人35例作為正常對(duì)照組。分離外周血單個(gè)核細(xì)胞,提取總RNA,逆轉(zhuǎn)錄合成cDNA,采用Real-time PCR檢測(cè),以得到的BDNF基因Ct值與GAPDH基因Ct值的差值作為外周血單個(gè)核細(xì)胞中BDNF mRNA相對(duì)表達(dá)含量,對(duì)頭痛組各組與正常對(duì)照組進(jìn)行比較。結(jié)果正常對(duì)照組與偏頭痛組、緊張型頭痛組、叢集性頭痛組、藥物過(guò)量性頭痛組比較外周血單個(gè)核細(xì)胞中的BDNF mRNA表達(dá)水平無(wú)差異表達(dá)。結(jié)論外周血單個(gè)核細(xì)胞中的BDNF mRNA表達(dá)水平與偏頭痛、緊張型頭痛、叢集性頭痛、藥物過(guò)量性頭痛無(wú)明顯相關(guān)。
[Abstract]:Background headache is one of the most frequent and common symptoms of pain in humans. Migraine, tension headache, cluster headache are common in primary headache. Drug overdose headache is a special and common type of headache in secondary headache. The aim of this study was to detect the expression of PACAPnBDNF in peripheral blood mononuclear cells (PBMC) of patients with headache and healthy people to explore the correlation between PACAPnBDNF and headache. To find a simple, feasible and accurate biological index for clinical diagnosis and differential diagnosis of headache. To provide a new theoretical basis for the study of the mechanism of headache. The thesis consists of two parts: part one: mRNA expression of PACAP in peripheral blood mononuclear cells of headache patients objective to study the PACAP mRNA table in peripheral blood mononuclear cells of headache patients. Methods 101 headache patients were selected from the headache clinic of PLA General Hospital from April 2014 to February 2015. According to the diagnostic criteria of I CHD-3 beta, they were divided into four groups: migraine group, tension headache group, cluster headache group and drug overdose headache group; 35 healthy persons were selected as normal control group. Peripheral blood mononuclear cells (PBMC) were isolated. Total RNAs were extracted, reverse transcription was used to synthesize cDNAs, and Real-time PCR was used to detect the difference between Ct value of PACAP gene and Ct value of GAPDH gene as the relative expression content of PACAP mRNA in peripheral blood mononuclear cells. Results the expression of PACAP mRNA in peripheral blood mononuclear cells in migraine group was significantly lower than that in normal control group (P 0.05). There was no difference in the expression of PACAP mRNA in peripheral blood mononuclear cells between cluster headache group and drug overdose headache group. Conclusion the expression of PACAP mRNA in peripheral blood mononuclear cells is closely related to migraine and is associated with tension headache. There is no obvious correlation between cluster headache and drug overdose headache. It is helpful to distinguish migraine from tension headache, cluster headache and drug overdose headache. Part 2: mRNA expression in peripheral blood mononuclear cells of headache patients objective to study the expression level of BDNF mRNA in peripheral blood mononuclear cells of headache patients on 2014. From April to February 2015, there were 101 headache patients in the headache clinic of PLA General Hospital. According to the ICHD-3 beta criteria, the patients were divided into four groups: migraine group, tension headache group, cluster headache group and drug overdose headache group. Conclusion the expression of BDNF mRNA in peripheral blood mononuclear cells is similar to migraine, tension headache and cluster headache. There was no significant correlation between drug overdose headache.
