有先兆偏頭痛患者的靜息態(tài)功能磁共振研究

【摘要】:目的:影像學研究已有發(fā)現(xiàn)無先兆偏頭痛患者的腦結(jié)構(gòu)及功能發(fā)生改變,本研究采用靜息態(tài)功能磁共振成像技術(shù)來探討有先兆偏頭痛患者的自發(fā)神經(jīng)元活動特征,分析偏頭痛靜息狀態(tài)腦功能網(wǎng)絡的變化,以便更好地認識有先兆偏頭痛的發(fā)病機制。 方法:對7例發(fā)作間期有先兆偏頭痛患者和7例年齡、性別及受教育程度相匹配的健康對照行靜息態(tài)功能磁共振成像掃描,采用DPARSFA2.2軟件分析原始數(shù)據(jù),得出全腦低頻振幅(amplitude of low frequencyfluctuation, ALFF),進行雙樣本t檢驗,并以ALFF差異的腦區(qū)為感興趣區(qū)(region of interest,ROI)校正后行功能連接(Functional connectivity,F(xiàn)C)分析。 結(jié)果: 1兩組局部腦區(qū)的ALFF值顯示在圖1及圖2(P0.05).在兩組的ALFF值均顯著增高的腦區(qū)有前后扣帶回皮層、楔前葉、下頂葉、內(nèi)側(cè)前額葉皮質(zhì)、島葉等。這些腦區(qū)均屬于默認網(wǎng)絡。 2與對照組相比,,病例組雙側(cè)額上回ALFF值顯著高于對照組(P0.05);與對照組相比,病例組左側(cè)眶額皮層ALFF值顯著高于對照組(P0.05);病例組高于對照組的T值最高點在左側(cè)額上回。 3與對照組相比,病例組左側(cè)顳中回ALFF值顯著低于對照組(P0.05);與對照組相比,病例組左側(cè)顳下回ALFF值顯著低于對照組(P0.05);與對照組相比,病例組左側(cè)尾狀核ALFF值顯著低于對照組(P0.05);與對照組相比,病例組雙側(cè)丘腦ALFF值顯著低于對照組(P0.05);與對照組相比,病例組右側(cè)運動前區(qū)ALFF值顯著低于對照組(P0.05);病例組低于對照組的T值最高點在左側(cè)顳中回。 4與對照組相比,病例組小腦、島葉、額葉、扣帶回、緣上回、腦干與相關腦區(qū)的功能連接發(fā)生改變,具體為:與對照組相比,病例組左側(cè)小腦、右側(cè)腦島、腦干與左側(cè)額上回之間的功能連接減弱,病例組右側(cè)眶額皮層、左側(cè)額中回、雙側(cè)前扣帶皮質(zhì)、右側(cè)緣上回與左側(cè)額上回功能連接增強。 結(jié)論:有先兆偏頭痛患者頭痛發(fā)作間期疼痛相關腦區(qū)功能異常。這些發(fā)現(xiàn)有助于理解有先兆偏頭痛的發(fā)病機制。
[Abstract]:Objective: imaging studies have found no changes in brain structure and function in patients with migraine aura. In this study, resting functional magnetic resonance imaging (fMRI) was used to investigate the characteristics of spontaneous neuronal activity in patients with migraine aura, and to analyze the changes of brain function network in patients with migraine. In order to better understand the pathogenesis of migraine aura. Methods: resting functional magnetic resonance imaging was performed in 7 patients with interictal migraine aura and 7 healthy controls with matched age, sex and education level. The DPARSFA2.2 software was used to analyze the original data and the low-frequency amplitude of low frequencyfluctuation was obtained. ALF FN, a double sample t test was performed, and the regions of interest were the regions of interest in which the ALFF difference was found in the brain regions. ROI) functional connectivity analysis was performed after correction. Results: 1the ALFF values of the local brain areas in both groups were shown in figs. 1 and 2 (P0.05). In both groups, there were anterior cingulate cortex, anterior cuneate lobe and inferior parietal lobe in brain regions with significantly increased ALFF values. Medial prefrontal cortex, insular lobe, and so on. These brain regions belong to the default network. 2Compared with the control group, the ALFF value of bilateral superior frontal gyrus in the case group was significantly higher than that in the control group (P 0.05). Compared with the control group, the ALFF value of the left orbital frontal cortex in the case group was significantly higher than that in the control group (P 0.05). The highest T value in the case group was higher than that in the control group in the left superior frontal gyrus. 3Compared with the control group, the ALFF value of the left middle temporal gyrus in the case group was significantly lower than that in the control group (P 0.05). Compared with the control group, the ALFF value of the left infratemporal gyrus in the case group was significantly lower than that in the control group (P 0.05). Compared with the control group, the ALFF value of the left caudate nucleus in the case group was significantly lower than that in the control group (P 0.05). Compared with the control group, the ALFF value of bilateral thalamus in the case group was significantly lower than that in the control group (P 0.05). Compared with the control group, the ALFF value of the right preexercise area in the case group was significantly lower than that in the control group (P 0.05). The highest T value in the case group was in the left middle temporal gyrus. 4Compared with the control group, the functional connections of cerebellum, insular lobe, frontal lobe, cingulate gyrus, superior marginal gyrus, brainstem and the related brain regions were changed in the case group, which were as follows: compared with the control group, the left cerebellum and the right cerebral island in the case group were changed. The functional connections between the brain stem and the left superior frontal gyrus were weakened. In the case group, the right orbital frontal cortex, the left middle frontal gyrus, the bilateral anterior cingulate cortex, the right superior marginal gyrus and the left superior frontal gyrus were enhanced. Conclusion: these findings are helpful to understand the pathogenesis of migraine with aura migraine.
