【摘要】:目的探討TCD聯(lián)合CTA及CT灌注成像在評(píng)估頸動(dòng)脈狹窄或閉塞患者腦血流動(dòng)力學(xué)變化中的價(jià)值。方法選取2013年4月至2015年10月94例頸動(dòng)脈狹窄或閉塞患者為研究對(duì)象,選取同期在我院進(jìn)行體檢的健康志愿者94例為對(duì)照組,所有患者及志愿者均采用TCD聯(lián)合CTA及CT灌注成像,比較不同狹窄程度患者與對(duì)照組的腦血流動(dòng)力學(xué)變化情況。結(jié)果輕度狹窄、中度狹窄、重度狹窄、閉塞患者的兩側(cè)大腦動(dòng)脈平均血流速度分別為(61.09±18.37)c m/s、(5 5.2 6±1 8.6 1)c m/s、(47.28±15.96)cm/s、(41.75±16.22)cm/s顯著低于對(duì)照組的(69.42±21.55)cm/s(P0.05)。輕度狹窄、中度狹窄、重度狹窄、閉塞患者的達(dá)峰時(shí)間分別為(8.24±2.39)s、(9.36±2.54)s、(10.17±2.92)s、(10.94±2.75)s顯著高于對(duì)照組的(7.63±2.12)s(P0.05)。結(jié)論頸內(nèi)動(dòng)脈狹窄或閉塞與顱內(nèi)動(dòng)脈血流動(dòng)力學(xué)、側(cè)支循環(huán)管開(kāi)放情況、腦灌注參數(shù)等密切相關(guān),TCD聯(lián)合CTA及CT灌注成像可以綜合評(píng)估頸動(dòng)脈狹窄或閉塞患者的腦血流動(dòng)力學(xué)變化,為制定合理有效的治療方案提供科學(xué)依據(jù)。
[Abstract]:Objective to evaluate the value of TCD combined with CTA and CT perfusion imaging in evaluating cerebral hemodynamic changes in patients with carotid artery stenosis or occlusion. Methods from April 2013 to October 2015, 94 patients with cervical movement were selected. Patients with stenosis or occlusion of veins were studied. 94 healthy volunteers who underwent physical examination in our hospital at the same time were selected as control group. All the patients and volunteers were treated with TCD combined with CTA and CT perfusion imaging. The changes of cerebral hemodynamics in patients with different degree of stenosis were compared with those in control group. Results mild stenosis, moderate stenosis and severe stenosis were observed.
