【摘要】：目的探討血小板與淋巴細胞比值(platelet to lymphocyte ratio, PLR)和中性粒細胞與淋巴細胞比值(neutrophil to lymphocyte ratio, NLR)在急性缺血性腦卒中(acute ischemic stroke, AIS)患者中的變化以及臨床意義。方法 收集2013年12月至2015年4月間AIS患者200例(A組)及同期健康體檢者50例(B組),檢測兩組患者的血常規(guī)指標,計算PLR和NLR,比較分析AIS患者和健康體檢者PLR和NLR值,根據(jù)AIS的嚴重程度全部患者分為輕度(A1)、中度(A2)和重度組(A3),比較分析AIS不同組間PLR和NLR值的變化。應用ROC曲線評價PLR和NLR的臨床預測價值。結(jié)果 A組PLR和NLR均值明顯高于B組(142.12±32.95 vs84.19±21.29,5.11±1.33vs 1.95±0.44,均P0.05)。A1-A3組PLR均值各為120.34±23.60,141.26±39.96和184.86±42.81,A1和A3組,A2和A3組比較有顯著性差異(均P0.05),A1-A3組NLR均值各為2.94±0.6,5.06±1.24,9.35±2.72,三組間比較均有顯著性差異(均P0.05)。等級相關(guān)性分析顯示疾病等級與NLR和PLR呈顯著正相關(guān)(相關(guān)系數(shù)各為r=0.698,r=0.365,P0.05)。ROC曲線下NLR和PLR的面積(AUC)各為0.82(0.77-0.89 95%C1,P0.05)和0.805(0.750-0.860 95%CI,P0.05,)；敏感度各為69%和53%,特異度均為98%。NLR和PLR切值各為2.81和124.7。結(jié)論 1.NLR和PLR對預測AIS患者病情嚴重程度有較高的參考價值。2.NLR和PLR對判斷AIS患者預后有較高的參考價值。
