北京市二價(jià)脊髓灰質(zhì)炎減毒活疫苗引入前后不同基礎(chǔ)免疫程序效果和疫苗效價(jià)分析

【摘要】:目的了解二價(jià)脊髓灰質(zhì)炎(脊灰)減毒活疫苗(b OPV)引入前后北京市2014年和2016年不同基礎(chǔ)免疫程序免疫效果和疫苗效價(jià)。方法 2014年和2016年脊灰疫苗基礎(chǔ)免疫程序分別為3劑三價(jià)脊灰減毒活疫苗(t OPV)全程免疫和1劑脊灰滅活疫苗(IPV)與2劑二價(jià)脊灰減毒活疫苗(b OPV)序貫免疫,分別在北京市兩個(gè)區(qū)選擇完成基礎(chǔ)免疫后4-8周的嬰兒,采集血標(biāo)本以檢測脊灰病毒中和抗體,同時(shí)采集不同疫苗儲(chǔ)運(yùn)環(huán)節(jié)的疫苗標(biāo)本以檢測疫苗效價(jià)。結(jié)果 2014年和2016年分別調(diào)查兒童72人和68人。2014年基礎(chǔ)免疫后Ⅰ、Ⅲ型脊灰病毒中和抗體陽性率分別為98.61%和100%,抗體滴度(1∶)[中位數(shù)(四分位數(shù)間距)]分別為1 536(1 024-1 536)、1 536(512-1 536);2016年基礎(chǔ)免疫后Ⅰ型、Ⅲ型脊灰病毒中和抗體陽性率均為100%,抗體滴度(1∶)分別為1 024(512-1 536)、1 536(1 024-1 536)。2014年和2016年Ⅰ、Ⅲ型脊灰病毒中和抗體分布均有顯著性差異(Ⅰ型:χ2=8.77,P=0.038;Ⅲ型:χ2=16.76,P=0.001)。2014年t OPV的平均效價(jià)為6.1±0.1Lg CCID50/劑,2016年b OPV的平均效價(jià)為6.9±0.2Lg CCID50/劑(t=-7.76,P0.001)。結(jié)論新疫苗b OPV引入后,IPV-b OPV序貫免疫程序的基礎(chǔ)免疫效果與引入前的t OPV全程和IPV-t OPV序貫免疫程序效果一樣好;疫苗儲(chǔ)運(yùn)各環(huán)節(jié)疫苗效價(jià)均合格,冷鏈運(yùn)轉(zhuǎn)良好。

Methods the basic immunization procedures of polio vaccine in 2014 and 2016 were 3 doses of trivalent live attenuated poliovirus vaccine (t OPV), 1 dose of poliomyelitis inactivated vaccine (IPV) and 2 doses of divalent live attenuated vaccine (b OPV) sequential immunization. Infants aged 4-8 weeks after basic immunization were selected in two districts of Beijing. Blood samples were collected to detect neutralizing antibodies against poliovirus and vaccine samples from different vaccine storage and transportation links were collected to detect vaccine titer.
