三七總皂苷殼聚糖溫控凝膠對大鼠拔牙創(chuàng)愈合以及BMP-2表達(dá)的影響

【摘要】:目的:由于醫(yī)學(xué)的有限性,目前臨床工作中有不少因各種局部或全身因素而不得不拔除患牙的情況,而在牙齒拔除后,拔牙創(chuàng)的愈合進(jìn)程直接影響患者能否盡早地佩戴義齒,并且拔牙窩改建后的局部牙槽嵴的形態(tài)和功能,對義齒的固位和穩(wěn)定、鄰牙的正畸移動以及人工種植牙的骨量條件等均產(chǎn)生影響,因此這一過程是口腔頜面外科、修復(fù)、正畸和種植醫(yī)生共同關(guān)注的重要課題。本研究通過動物實(shí)驗(yàn)旨在探討載中藥三七總皂苷殼聚糖溫控凝膠對大鼠拔牙創(chuàng)愈合的影響,能否有效控制剩余牙槽嵴的吸收,以及檢測其對BMP-2表達(dá)的影響。方法:3月齡SD雄性大鼠(體重200~250g)48只,分別在大鼠拔牙前9天、6天、3天使用便攜式牙鉆機(jī)將其左下頜中切牙齊齦緣磨除牙冠,使其左下中切牙拔牙前不能與對頜牙建立咬合接觸關(guān)系,在模型建立階段死了三只。在第十天將剩余45只大鼠分為五組(高濃度組、中濃度組、低濃度組、空白凝膠組、完全空白組),將左下頜中切牙完整拔除后,分別向前四組大鼠拔牙創(chuàng)內(nèi)置入提前配制的三七總皂苷殼聚糖凝膠高濃度(2.8mg/ml)、中濃度(1.4mg/ml)、低濃度(0.7mg/ml)、空白凝膠各20ul,空白對照組不放置任何東西。各組大鼠分別于術(shù)后7、14、28天后隨機(jī)選擇三只進(jìn)行心臟灌注固定方法處死。剝離大鼠左右下頜骨,拍左右下頜骨數(shù)字牙片,測量各組剩余牙槽嵴的相對長度;制作病理切片,通過HE染色觀察拔牙創(chuàng)愈合情況以及免疫組織化學(xué)染色檢測各組不同時期拔牙創(chuàng)BMP-2的表達(dá),統(tǒng)計(jì)分析。結(jié)果:1.各組剩余牙槽嵴相對長度的統(tǒng)計(jì)結(jié)果:7天時,五組數(shù)值組間相比,均無統(tǒng)計(jì)學(xué)差異。14天和28天時,3個載藥組與空白凝膠組、完全空白組比,3個載藥組數(shù)值大于空白凝膠組、完全空白組,均有統(tǒng)計(jì)學(xué)差異;其中,中濃度組數(shù)值大于高濃度組和低濃度組,有統(tǒng)計(jì)學(xué)意義;而高濃度組與低濃度組比,空白凝膠組與完全空白組相比,數(shù)值均無統(tǒng)計(jì)學(xué)差異。同組在不同時間點(diǎn),五組剩余牙槽嵴相對長度,第7d與第14d相比,無統(tǒng)計(jì)學(xué)差異,但第28天數(shù)值均小于第7天和第14天,有統(tǒng)計(jì)學(xué)差異。2.各組BMP-2陽性細(xì)胞計(jì)數(shù)統(tǒng)計(jì)結(jié)果:在這3個時間點(diǎn)3個載藥組與空白凝膠組、完全空白組比,3個載藥組數(shù)目多于空白凝膠組、完全空白組,均有統(tǒng)計(jì)學(xué)差異。在這3個時間點(diǎn)3個載藥組相比,中濃度組數(shù)目多于高組和低組,有統(tǒng)計(jì)學(xué)意義。3個時間點(diǎn)高濃度組與低濃度組比,空白凝膠組與完全空白組相比,數(shù)目均無統(tǒng)計(jì)學(xué)差異。同組在不同時間點(diǎn),五組陽性細(xì)胞計(jì)數(shù)峰值均在第14天,第7d與第28d相比,無統(tǒng)計(jì)學(xué)差異。3.HE染色觀察各組成骨狀況與各組免疫組化結(jié)果相符。結(jié)論:三七總皂苷殼聚糖溫控凝膠能加快牙槽窩的愈合進(jìn)程,提升BMP-2的表達(dá)水平,抑制牙槽骨的吸收;三七總皂苷在中濃度(1.4mg/ml)應(yīng)用時其作用性能優(yōu)于高濃度組(2.8mg/ml)和低濃度組0.7mg/ml);殼聚糖溫控凝膠具有一定的成骨誘導(dǎo)作用,但是單獨(dú)應(yīng)用效果并不明顯。
[Abstract]:Objective: Because of the limitation of medicine, there are not many cases in the clinical work because of various local or systemic factors, and after the tooth extraction, the healing process of tooth extraction can directly affect the ability of the patient to wear the denture as soon as possible. and the shape and function of the partial tooth groove after the reconstruction of the tooth extraction socket have the influence on the retention and the stability of the dentures, the orthodontic movement of the adjacent teeth and the bone volume conditions of the artificial planting teeth, so that the process is the oral and maxillofacial surgery and the repair, Orthodontic and planting doctors are an important subject of common concern. The purpose of this study was to investigate the effect of chitosan-controlled gel on the healing of tooth extraction in rats, and to control the absorption of residual tooth and its effect on the expression of BMP-2. Methods: Forty-eight SD male rats (200 to 250g in body weight) were used to remove the dental crown from the gingival margin of the left and lower mandibular incisors for 9 days, 6 days and 3 days before the tooth extraction in the rat. Three dead in the model building phase. the remaining 45 rats were divided into five groups (high concentration group, medium concentration group, low concentration group, blank gel group and complete blank group) on the tenth day, and the left lower jaw and the left lower jaw were completely extracted, The high concentration (2. 8mg/ ml), medium concentration (1. 4mg/ ml), low concentration (0. 7mg/ ml), and 20ul of the blank gel and the blank control group were not put in the control group, respectively. Three groups of rats were randomly selected for cardiac perfusion fixation after 7, 14 and 28 days after operation. The relative length of the left and right mandibles of the rat was dissected, the relative length of the remaining tooth grooves in each group was measured, the pathological sections were made, the healing of the tooth extraction was observed by HE staining, and the expression of BMP-2 was detected by immunohistochemical staining, and the statistical analysis was carried out. Results: 1. The statistical results of the relative length of the remaining teeth in each group were as follows: no statistical difference was found between group 5 and group 5 on day 7 and day 28, and there was no statistical difference between the three drug-carrying groups and the blank gel group, the total blank group and the blank group, and the total blank group was all blank. The value of medium concentration group was higher than that of the high concentration group and the low concentration group, and there was no statistical difference between the high concentration group and the low concentration group, and the blank gel group was not statistically different from the complete blank group. There was no statistical difference in the relative length of the remaining teeth in the five groups at different time points in the same group, but the 28-day value was less than the 7th and 14th days, and there was a statistical difference. The results of the counting of BMP-2 positive cells in each group showed that there was a statistically significant difference between the three drug-carrying groups and the blank gel group, the total blank group ratio, the number of the three drug-carrying groups, and the total blank group. There were no significant differences in the number of concentration groups in the three groups compared with the high and low groups, and there was no statistical difference between the group of high concentration and the low concentration group at the 3 time points, and the blank group was not statistically different from the complete blank group. At different time points, the peak of the five-group positive cell counts was on the 14th day, and no statistical difference was found in the 7th day and the second 28d. Conclusion: The chitosan-chitosan temperature-controlled gel can accelerate the healing process of the alveolar fossa, improve the expression level of BMP-2, and inhibit the absorption of the alveolar bone. The effect of the total soap of the pseudo-ginseng is better than that of the high-concentration group (2.8mg/ ml) and the low-concentration group (0.7mg/ ml). The chitosan temperature-controlled gel has a certain bone-inducing effect, but the effect of the individual application is not obvious.
