【摘要】：目的:定量研究嵌體和全冠修復(fù)根管治療后磨牙與前磨牙牙齒外形改變量,以及對(duì)牙周組織的影響。方法:選取需要根管治療的牙周組織健康和慢性牙周炎的患牙各30例,完善根管治療后,隨機(jī)各取15例進(jìn)行嵌體或全冠修復(fù)。口外掃描儀掃描同一患牙修復(fù)前后石膏模型,Geomagic Qualify 12.0將三維模型數(shù)據(jù)兩兩進(jìn)行最佳擬合對(duì)齊和3D比較,計(jì)算配對(duì)數(shù)據(jù)間的平均偏差(averaged errors,AE)。ELASA法定量檢測(cè)治療前后齦溝中液彈性蛋白酶(elastase,EA)及前列腺素E2(prostaglandin E2,PGE2)變化。結(jié)果:1.用全冠修復(fù)的患牙外形形態(tài)于頰側(cè)近合面1/2,近齦端1/2及舌側(cè)近齦端1/2改變量大于嵌體修復(fù)患牙的改變量,并具有統(tǒng)計(jì)學(xué)差異(P0.05)。2.慢性牙周炎患牙經(jīng)牙周基礎(chǔ)治療一年后齦溝液中前列腺素E2和彈性蛋白酶濃度明顯降低,并具有統(tǒng)計(jì)學(xué)意義(P0.05);牙周健康患牙無(wú)明顯改變。結(jié)論:全冠修復(fù)對(duì)牙齒原有外形在頰舌側(cè)改變量大于嵌體修復(fù),但并未對(duì)牙周組織產(chǎn)生明顯影響。慢性牙周病患牙接受牙周基礎(chǔ)治療后,牙周狀態(tài)明顯改善。
[Abstract]:Objective: to study quantitatively the changes of tooth shape and the effect on periodontal tissue of molar and premolar after root canal restoration with inlay and full crown. Methods: 30 cases of healthy periodontal tissue and 30 cases of chronic periodontitis were selected. 15 cases were randomly selected for inlay or full crown repair after perfect root canal therapy. Geomagic Qualify 12.0 scanned the same tooth before and after the restoration of the same tooth. The 3D model data were matched and compared. The mean deviation (averaged errors,AE). ELASA method was used to measure the changes of elastase (elastase,EA) and prostaglandin E 2 (prostaglandin E 2 PGE 2) in gingival sulcus before and after treatment. The result is 1: 1. The shape of the affected teeth repaired with full crown was 1 / 2 in the proximal side of the cheek. The changes of the proximal gingival end 1 / 2 and the lingual side of the gingival end were larger than that of the inlay restoration tooth, and the difference was statistically significant (P0.05). The concentration of prostaglandin E _ 2 and elastase in gingival crevicular fluid of chronic periodontitis was significantly decreased after one year of periodontal basic treatment (P0.05). Conclusion: the change of tooth shape in buccal and lingual side is larger than that in inlay repair, but it has no obvious effect on periodontal tissue. The periodontal state of chronic periodontal disease patients improved significantly after periodontal basic treatment.
