【摘要】：目的探究老年慢性牙周炎患者齦溝液(GCF)中基質(zhì)金屬蛋白酶(MMP)-1和MMP組織抑制物(TIMP)-1水平與齦下刮治和根面平整術(shù)效果和牙周組織狀況的關(guān)系。方法 53例齦下刮治和根面平整術(shù)治療的老年慢性牙周炎患者為觀察組,49例行健康檢查的老年人為對照組。對比治療前后兩組牙周指標(biāo)菌斑指數(shù)(PI)、探針出血(BOP)、探針深度(PD)、附著喪失(AL)及GCF中MMP-1和TIMP-1水平差異變化;分析觀察組GCF中MMP-1、TIMP-1與PI的相關(guān)性。結(jié)果兩組治療前牙周指標(biāo)及GCF中MMP-1、TIMP-1水平差異均顯著,且治療后觀察組牙周指標(biāo)及GCF中MMP-1、TIMP-1水平與對照組及治療前差異顯著,觀察組GCF中MMP-1(r=0.493,P0.001)及TIMP-1(r=0.515,P0.001)與PI呈正相關(guān)。結(jié)論老年慢性牙周炎患者經(jīng)齦下刮治和根面平整術(shù)治療后效果顯著,牙周組織現(xiàn)狀好轉(zhuǎn)改善,GCF中MMP-1、TIMP-1水平下降明顯,炎癥反應(yīng)逐漸消失。
[Abstract]:Objective to investigate the relationship between the levels of matrix metalloproteinase-1 (MMP-1) and tissue inhibitor of MMP (TIMP- 1) in gingival crevicular fluid (GCF) of elderly patients with chronic periodontitis and the effect of subgingival curettage, root surface leveling and periodontal tissue status. Methods 53 cases of senile chronic periodontitis treated by subgingival curettage and root surface leveling were treated as control group (n = 49). The levels of MMP-1 and TIMP-1 in GCF and MMP-1 and TIMP-1 in GCF were compared before and after treatment, and the correlation between TIMP-1 and Pi in GCF was analyzed. Results there were significant differences between the two groups in the periodontal indexes and the levels of MMP-1 and TIMP-1 in GCF, and the levels of MMP-1 and TIMP-1 in GCF in the observation group were significantly higher than those in the control group and before treatment. There was a positive correlation between the levels of MMP-1 and TIMP-1p0.001 in GCF and TIMP-1r0.515P0.001 in the observation group. Conclusion the effect of subgingival curettage and root surface leveling in elderly patients with chronic periodontitis was remarkable. The improvement of periodontal tissue and improvement of the level of MMP-1 and TIMP-1 in GCF decreased significantly, and the inflammatory reaction gradually disappeared.
