【摘要】：目的探討老年冠心病伴重度牙周炎患者不同牙周治療后,牙周狀況及血清腫瘤壞死因子(TNF-α)、白細(xì)胞介素-6(IL-6)和C反應(yīng)蛋白(CRP)的變化。方法將90例伴有重度牙周炎的老年冠心病患者分為3組,每組30例,對(duì)照組僅進(jìn)行冠心病治療,基礎(chǔ)組在冠心病治療基礎(chǔ)上進(jìn)行牙周基礎(chǔ)治療,聯(lián)合組患者在冠心病治療基礎(chǔ)上進(jìn)行牙周基礎(chǔ)治療和翻瓣術(shù),比較3組各觀察指標(biāo)的差異。結(jié)果聯(lián)合組和基礎(chǔ)組經(jīng)牙周基礎(chǔ)治療后,牙周袋深度(PD)、齦溝出血指數(shù)(SBI)、斑指數(shù)(PLI)、探診深度、探診出血均明顯下降(P0.05),并有顯著的附著獲得(P0.05);聯(lián)合組與基礎(chǔ)組治療后比較,PD、SBI、臨床附著喪失(CAL)、探診深度、探診出血差異有統(tǒng)計(jì)學(xué)意義(P0.05)。治療后,聯(lián)合組和基礎(chǔ)組TNF-α、IL-6及CRP水平較對(duì)照組明顯降低(P0.05);基礎(chǔ)組和聯(lián)合組治療后血清中TNF-α、IL-6及CRP水平比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論牙周治療和翻瓣術(shù)均能降低老年冠心病患者相關(guān)血清炎性因子TNF-α、IL-6和CRP水平,但相對(duì)于牙周基礎(chǔ)治療,翻瓣術(shù)效果更佳。
[Abstract]:Objective to investigate the changes of periodontal status and serum tumor necrosis factor TNF- 偽 (TNF- 偽), interleukin-6 (IL-6) and C-reactive protein (CRP) in elderly patients with coronary heart disease (CHD) and severe periodontitis after different periodontal treatments. Methods 90 elderly patients with severe periodontitis were divided into 3 groups, 30 cases in each group. The control group was treated with coronary heart disease only, and the basic group was treated with periodontal basic therapy on the basis of coronary heart disease. The patients in the combined group were treated with periodontal basic therapy and valvus operation on the basis of coronary heart disease, and the differences of the observed indexes among the three groups were compared. Results after periodontal basic treatment, the periodontal pocket depth, gingival sulcus bleeding index, plaque index and probing depth were observed in the combined group and the basic group. Compared with the basic group, the clinical attachment loss of CALI, the depth of probing and the difference of the bleeding between the combined group and the basic group were statistically significant (P 0.05). After treatment, the levels of TNF- 偽 IL-6 and CRP in the combined group and the basic group were significantly lower than those in the control group, while the serum levels of TNF- 偽 IL-6 and CRP in the basic group and the combined group were significantly lower than those in the control group (P 0.05). Conclusion both periodontal therapy and valvuloplasty can decrease the levels of TNF- 偽 IL-6 and CRP in serum of elderly patients with coronary heart disease, but the effect of valvulation is better than that of basic periodontal therapy.
