四種再礦化制劑對(duì)酸蝕牙釉質(zhì)和酸蝕牙釉質(zhì)磨損的影響

【摘要】:背景牙齒酸蝕癥是在無細(xì)菌參與的情況下,接觸牙面的酸或其螯合物的化學(xué)侵蝕及溶解作用導(dǎo)致牙體硬組織的喪失。牙齒磨損是指主要由機(jī)械摩擦作用造成的牙體硬組織漸進(jìn)性喪失。酸蝕牙齒磨損是指牙酸蝕和磨損同時(shí)發(fā)生,與酸蝕和磨損單獨(dú)發(fā)生相比,酸蝕牙齒磨損會(huì)導(dǎo)致牙體更為嚴(yán)重的破壞。 目前,預(yù)防酸蝕磨損的方法一般包括改變飲食習(xí)慣減少飲食中的酸對(duì)牙齒的侵蝕、避免與酸性環(huán)境接觸、使用增強(qiáng)牙齒抗酸能力的藥物等等。在日常生活中,氟化物的使用最為廣泛。酪蛋白磷酸肽-無定CPP-ACP)形磷酸鈣復(fù)合物(Casein phosphopeptide-amorphous calciumphosphate,是從牛奶中提取的不含氟離子的制劑,其對(duì)于牙體硬組織作用的主要機(jī)理在于促進(jìn)牙體硬組織再礦化和抑制脫礦。CPP-ACP與氟離子結(jié)合可生成含氟酪蛋白磷酸肽-無定形磷酸鈣復(fù)合物(caseinphosphopeptide-amorphous calcium phosphate fluoride, CPP-ACPF)。含氟磷酸三鈣(Tri-calcium phosphate fluoride,TCP+F)富含鈣、磷、氟離子,可以阻止牙齒脫礦,幫助牙釉質(zhì)修復(fù)和再礦化。 綜合大量國內(nèi)外的研究發(fā)現(xiàn),現(xiàn)在國內(nèi)外有關(guān)多種再礦化制劑防治牙酸蝕與磨損的報(bào)道很少。本研究借鑒國內(nèi)外相關(guān)學(xué)者的研究方法,旨在比較市場上的4種再礦化制劑——CPP-ACP、CPP-ACPF、TCP+F、Duraphat保護(hù)漆對(duì)酸蝕牙釉質(zhì)和酸蝕牙釉質(zhì)磨損的影響及作用,為兒童及青少年牙齒酸蝕癥和牙齒酸蝕磨損的預(yù)防提供一個(gè)臨床參考。 目的比較CPP-ACP、CPP-ACPF、TCP+F、Duraphat保護(hù)漆4種再礦化制劑對(duì)酸蝕乳牙和恒牙釉質(zhì)表面硬度和表面形態(tài)的影響,以及對(duì)酸蝕乳牙和恒牙釉質(zhì)刷牙磨損后表面硬度和表面形態(tài)的影響。 材料和方法1.四種再礦化制劑對(duì)酸蝕牙釉質(zhì)影響的比較研究:選擇因滯留拔除的下頜乳中切牙和因阻生拔除的下頜第三磨牙分別制成釉質(zhì)塊,分別選擇硬度值介于280-330HV的釉質(zhì)塊50個(gè),隨機(jī)分為5組。將5組樣本進(jìn)行可口可樂酸蝕處理,5次/d,5min/次,共5天。酸蝕完成后5組樣本分別涂布去離子水、CPP ACP、CPP-ACPF、TCP+F、Duraphat,繼續(xù)礦化-酸蝕處理5天。采用顯微硬度法測(cè)量處理前后牙釉質(zhì)表面硬度,采用掃描電鏡觀察牙釉質(zhì)表面形態(tài)的變化。 2.四種再礦化制劑對(duì)酸蝕牙釉質(zhì)磨損影響的比較研究:選擇因滯留拔除的下頜乳中切牙和因阻生拔除的下頜第三磨牙分別制成釉質(zhì)塊,分別選擇硬度值介于280-330HV的釉質(zhì)塊50個(gè),隨機(jī)分為5組。將5組樣本進(jìn)行可口可樂酸蝕處理,5次/d,5min/次,共5天。采用電動(dòng)牙刷對(duì)酸蝕后的5組釉質(zhì)塊進(jìn)行刷牙磨損,刷牙所用的壓力為2N,2次/d,3min/次,共5天。刷牙間隔期5組樣本分別涂布去離子水、CPP ACP、CPP-ACPF、TCP+F、Duraphat,繼續(xù)磨損-再礦化-酸蝕處理5天。采用顯微硬度法測(cè)量處理前后牙釉質(zhì)表面硬度,采用掃描電鏡觀察牙釉質(zhì)表面形態(tài)的變化。 結(jié)果1. CPP ACP組、CPP-ACPF組、TCP+F組、Duraphat組4組顯微硬度值均高于去離子水組(P<0.05),但4組間差異無統(tǒng)計(jì)學(xué)意義(P>0.05)。掃描電鏡顯示除去離子水組以外,其余4組釉質(zhì)樣本均出現(xiàn)了大量礦物質(zhì)的沉積。 2.CPP ACP組、CPP-ACPF組顯微硬度值繼續(xù)降低,與去離子水組無顯著差異(P>0.05); TCP+F組、Duraphat組顯微硬度值出現(xiàn)增加,顯著高于去離子水組(P<0.05)。掃描電鏡結(jié)果顯示CPP-ACP、CPP-ACPF處理后的牙釉質(zhì)表面可見少許礦物質(zhì)沉積。經(jīng)TCP+F、Duraphat處理后的牙釉質(zhì)表面變得平整光滑。 結(jié)論1. CPP ACP、CPP-ACPF、TCP+F、Duraphat均能有效抑制酸蝕牙釉質(zhì)的持續(xù)脫礦。 2. Duraphat和TCP+F可以防止持續(xù)酸蝕牙釉質(zhì)的磨損。
[Abstract]:background dental erosion is the loss of dental hard tissue caused by chemical attack and dissolution of acid or a chelate thereof without the participation of bacteria . The tooth wear refers to the progressive loss of tooth body hard tissue caused by mechanical friction .
Currently , methods for preventing acid erosion generally include changing eating habits to reduce acid - to - tooth erosion in the diet , avoid exposure to acidic environments , use of drugs that enhance tooth acid resistance , etc . In daily life , the most extensive use of fluoride - free CPP - ACP - shaped calcium phosphate complex ( CPP - ACP ) - free CPP - ACP ) - calcium phosphate complex ( CPP - ACP ) binds fluoride ions to produce a caseinphosphonite - amorphous calcium phosphate fluoride ( CPP - ACPF ) . Tri - calcium phosphate fluoride ( TCP + F ) is rich in calcium , phosphorus and fluoride ions , which can prevent tooth from demineralizing and help enamel repair and remineralization .
At home and abroad , it is found that there are few reports on the prevention and treatment of dental erosion and wear in many remineralization formulations at home and abroad . This study is aimed to compare the effects and effects of four remineralizations _ CPP - ACP , CPP - ACPF , TCP + F , Duraphat protective lacquer on the wear of acid - etched enamel and acid - etched enamel in the market . It provides a clinical reference for the prevention of dental erosion and abrasion of teeth in children and adolescents .
Objective To compare the effects of CPP - ACP , CPP - ACPF , TCP + F , Duraphat protective lacquer on the surface hardness and surface morphology of acid - etched deciduous teeth and permanent teeth enamel , and the effects of CPP - ACP , CPP - ACPF , TCP + F , Duraphat protective lacquer on the surface hardness and surface morphology of teeth after tooth erosion .
