臨床折裂的全瓷修復(fù)體斷口形貌分析

【摘要】:目的:針對臨床收集的全瓷修復(fù)體折裂片,應(yīng)用斷口表面形貌學(xué)進行分析全瓷失效的裂紋起始源和有關(guān)缺陷,探究全瓷修復(fù)體的實際破壞方式,為改進臨床設(shè)計和修復(fù)工藝提供依據(jù)。 方法:收集2012年11月至2013年11月于上海交通大學(xué)醫(yī)學(xué)院附屬第九人民醫(yī)院修復(fù)科就診的患者中折裂的全瓷修復(fù)體(10例)。1.采用X射線熒光光譜儀,多晶X射線粉末衍射儀檢測全瓷修復(fù)體的成分及結(jié)構(gòu)。2.采用立體顯微鏡觀察全瓷折裂片斷面整體形貌。3.采用掃描電子顯微鏡(SEM)觀察裂紋形態(tài)、斷口表面形貌、結(jié)構(gòu)缺陷等。 結(jié)果:循環(huán)疲勞是全瓷修復(fù)體折裂的重要原因,,不同的全瓷材料,主要的疲勞損傷模式不同。飾瓷內(nèi)部、飾瓷-核瓷交界面、連接區(qū)與飾瓷交界面的缺陷以及咬合接觸區(qū)的磨損等均能降低陶瓷材料的強度,從而導(dǎo)致全瓷修復(fù)體的失效。 結(jié)論:循環(huán)疲勞損傷是全瓷修復(fù)體折斷的主要原因,其中放射狀裂紋是修復(fù)體失效的主要模式,對瓷層的厚度高度敏感;飾瓷-核瓷界面是雙層全瓷修復(fù)體的薄弱環(huán)節(jié);結(jié)構(gòu)缺陷和異常咬合導(dǎo)致的應(yīng)力集中降低材料的強度,是修復(fù)體折斷潛在的危險因素。連接體的面積也是影響修復(fù)體臨床失效的重要原因。
[Abstract]:Objective: to analyze the crack origin and related defects of all-ceramic prosthesis by using fracture surface morphology, and to explore the actual failure mode of all-ceramic prosthesis. To provide basis for improving clinical design and repair process. Methods: from November 2012 to November 2013, 10 patients with broken all porcelain prosthesis were collected from the Department of Restoration of the Ninth people's Hospital affiliated to Shanghai Jiaotong University Medical College, Shanghai Jiaotong University. X ray fluorescence spectrometer and polycrystalline X ray powder diffractometer were used to detect the composition and structure of all ceramic restoration. A stereoscopic microscope was used to observe the overall morphology of the fracture section of all porcelain. The crack morphology, fracture surface morphology and structural defects were observed by scanning electron microscopy (SEM). Results: cyclic fatigue is an important reason for fracture of all ceramic restorations. The main fatigue damage modes are different for different all-ceramic materials. The strength of ceramic materials can be reduced, such as the interior of porcelain, the interface between porcelain and core, the defect of interface between porcelain and porcelain, and the wear of bite contact area, which leads to the failure of all ceramic restoration. Conclusion: cyclic fatigue damage is the main cause of fracture of all-ceramic prosthesis, in which radial crack is the main failure mode of restoration, highly sensitive to the thickness of porcelain layer, and the interface between porcelain and core porcelain is the weak link of double-layer all-ceramic prosthesis. The stress concentration caused by structural defects and abnormal bite reduces the strength of the material and is a potential risk factor for the fracture of restorations. The area of connective body is also an important reason for clinical failure of prosthesis.
