【摘要】：背景：隨著中國人口老齡化進(jìn)程的加快，無牙頜患者逐漸增多，為提高老年人的生活質(zhì)量，保證其身心健康，擁有一副良好功能的全口義齒是必不可少的。行全口義齒再修復(fù)的患者多為老年人，多伴有骨質(zhì)疏松，隨著戴用義齒及戴用時(shí)間的延長，會(huì)引起牙槽骨不斷吸收，導(dǎo)致牙槽骨變窄或者低平，甚至導(dǎo)致頜骨萎縮或變形，進(jìn)而影響全口義齒的固位和穩(wěn)定。且義齒咬合面的磨耗、人工牙缺損、變色等也影響患者的外貌及心理。臨床行義齒再修復(fù)患者也大都是以上述原因?yàn)橹髟V要求重新修復(fù)治療。而這類患者的舊義齒就好比患者個(gè)性化的“口腔檔案”，從中可以獲得許多供義齒再制參考的有用信息。 目的：探討舊義齒在臨床全口義齒再修復(fù)治療過程中的應(yīng)用價(jià)值，并使其在臨床工作中發(fā)揮臨床意義。 方法：選取大連醫(yī)科大學(xué)附屬口腔醫(yī)院2013年3月至2014年3月的全口義齒再修復(fù)病例6例，通過病史采集及對患者舊義齒的檢查分析，充分把握義齒再修復(fù)中需要注意的問題，制定治療計(jì)劃，完成全口義齒再修復(fù)。修復(fù)完成后以臨床問卷的形式對患者進(jìn)行臨床滿意度調(diào)查。 結(jié)果：調(diào)查結(jié)果顯示參考舊義齒制作的新義齒在固位穩(wěn)定性、舒適度、美觀性及咀嚼效率等各方面均較之前舊義齒有明顯改善。作為本研究對象的所有6例患者初戴義齒時(shí)均無無明顯異物感，表現(xiàn)出良好的適應(yīng)性，患者主觀滿意度高。 結(jié)論：全口義齒再修復(fù)過程中，臨床醫(yī)生不應(yīng)忽視對舊義齒的分析利用，需結(jié)合患者主訴、口腔檢查及對舊義齒的檢查分析，合理制定治療計(jì)劃，，再制作時(shí)注意保留舊義齒的合理設(shè)計(jì)，糾正和改進(jìn)舊義齒設(shè)計(jì)與制作過程中存在的問題與不足，使再制義齒既可恢復(fù)患者口腔功能又易被患者接受和適應(yīng)，提高臨床修復(fù)效果。
[Abstract]:Background: with the rapid aging of the population in China, the number of edentulous patients is increasing gradually. In order to improve the quality of life of the elderly and ensure their physical and mental health, it is necessary to have a functional complete denture. Most of the patients undergoing complete denture rerepair are elderly people, and most of them are accompanied by osteoporosis. With the extension of denture wear and wear time, the alveolar bone will continue to absorb, leading to narrow or low alveolar bone, or even to atrophy or deformation of the jaw bone. Thus, the retention and stability of complete denture are affected. The attrition of denture occlusal surface, artificial tooth defect and discoloration also affect the patient's appearance and psychology. The patients with clinical denture re-repair are mostly based on the above-mentioned reasons for the request of re-repair treatment. And this kind of patient's old denture is like patient's individualized "oral file", from which can obtain a lot of useful information for denture remaking reference. Objective: to explore the application value of old denture in the course of clinical complete denture rerepair and to make it play a clinical significance in clinical work. Methods: from March 2013 to March 2014, 6 cases of complete denture reconstruction were selected from the affiliated Stomatology Hospital of Dalian Medical University. Through the collection of medical history and the examination and analysis of the patients' old dentures, the problems needing attention in the reconstruction of dentures were fully grasped. Make treatment plan, complete complete denture restoration. The clinical satisfaction was investigated by clinical questionnaire after repair. Results: the results showed that the stability, comfort, beauty and mastication efficiency of the new dentures made with reference to the old dentures were significantly improved than those of the old dentures. All the 6 patients who were included in this study had no obvious foreign body feeling when they first wore dentures and showed good adaptability and high subjective satisfaction. Conclusion: clinicians should not ignore the analysis and utilization of old dentures in the process of complete denture rerepair, and should make a reasonable treatment plan by combining with the main complaint of patients, oral examination and examination and analysis of old dentures. Attention should be paid to the reasonable design of the old denture and the problems and deficiencies in the design and manufacture of the old denture should be corrected and improved, so that the remade denture could not only restore the oral function of the patients but also be easily accepted and adapted by the patients, and improve the effect of clinical restoration.
