【摘要】：目的:應(yīng)用磁共振波譜(1 H-MRS)技術(shù),探討血管性非癡呆認(rèn)知障礙(VCIND)患者的顱內(nèi)物質(zhì)代謝變化與認(rèn)知障礙的關(guān)系。方法:對25例VCIND患者(A組)及23例腦梗死后認(rèn)知功能正常(NCI,B組)進行簡易精神狀態(tài)檢查量表(MMSE)和蒙特利爾認(rèn)知評分量表(MoCA)評分,并對視空間及執(zhí)行功能進行評分。根據(jù)認(rèn)知功能評分結(jié)果,將兩組患者進行1 H-MRS檢查,測定雙側(cè)顳葉N-乙酰天冬氨酸(NAA)及膽堿復(fù)合物(Cho)與肌酸(Cr)比值,并探討A、B兩組物質(zhì)代謝比值與MoCA評分、視空間及執(zhí)行功能間的相關(guān)性。結(jié)果:與B組(NCI)NAA/Cr(左顳葉1.52±0.071、右顳葉1.51±0.06)相比,A組患者NAA/Cr(左顳葉1.37±0.08、右顳葉1.42±0.06)均下降(P0.05),A組顳葉NAA/Cr值下降程度與MoCA評分、視空間及執(zhí)行功能評分呈正相關(guān);A、B組間Cho/Cr比值變化無統(tǒng)計學(xué)意義。結(jié)論:1 H-MRS檢測顳葉代謝物變化對VCIND的早期診斷有重要作用。
[Abstract]:Aim: to investigate the relationship between intracranial substance metabolism and cognitive impairment in patients with vascular non-dementia cognitive impairment (VCIND) by magnetic resonance spectroscopy (1-H-MRS). Methods: 25 patients with VCIND (group A) and 23 patients with normal cognitive function after cerebral infarction (NCI,B group) were assessed with (MMSE) and (MoCA). The visual space and executive function were evaluated. According to the results of cognitive function score, 1 H-MRS was performed in the two groups. The ratio of N-acetylaspartic acid (NAA) and choline complex (Cho) to creatine (Cr) in bilateral temporal lobe was measured. The correlation between the ratio of substance metabolism and MoCA score, visual space and executive function between group B and group B. Results: compared with group B (left temporal lobe 1.52 鹵0.071, right temporal lobe 1.51 鹵0.06), NAA/Cr (left temporal lobe 1.37 鹵0.08, right temporal lobe 1.42 鹵0.06) was significantly decreased in group A (P0.05), (NCI) NAA/Cr in group A was significantly lower than that in group B (1.52 鹵0.071 in left temporal lobe and 1.51 鹵0.006 in right temporal lobe). In group A, the decrease of NAA/ Cr in temporal lobe was positively correlated with MoCA score, visual space and executive function score. There was no significant difference in the ratio of Cho/Cr between groups A and B. Conclusion: 1Detection of temporal lobe metabolites by H-MRS plays an important role in the early diagnosis of VCIND.
