【摘要】：目的了解北京市豐臺區(qū)社區(qū)精神分裂癥患者的外顯攻擊行為及影響因素。方法采用典型抽樣法選取豐臺社區(qū)為樣本地區(qū),收集2011年5月—2015年5月北京市精神衛(wèi)生信息管理系統(tǒng)(BMHIMS)中歸屬于該社區(qū)的精神分裂癥患者的檔案資料,包括人口統(tǒng)計學資料、臨床信息及行為相關資料。采用外顯攻擊行為量表(MOAS)評價患者的外顯攻擊行為發(fā)生情況,以總分=0分為無外顯攻擊行為,以總分0分為發(fā)生外顯攻擊行為。結果 2011—2015年豐臺社區(qū)居民的精神分裂癥患病率、發(fā)病率及精神分裂癥患者的外顯攻擊行為發(fā)生率比較,差異有統(tǒng)計學意義(P0.05)。其中,精神分裂癥患者的外顯攻擊行為發(fā)生率從2011年的27.1%升高至2015年的35.7%。2015年豐臺社區(qū)共有BMHIMS在檔精神分裂癥患者616例,其中220例(35.7%)發(fā)生外顯攻擊行為。是否發(fā)生外顯攻擊行為患者的平均年齡、受教育程度、軀體疾病發(fā)生情況、精神殘疾發(fā)生情況、陽性癥狀評分、陰性癥狀評分、一般精神病理學癥狀評分、服藥依從性比較,差異有統(tǒng)計學意義(P0.05)。多元線性回歸分析結果顯示,受教育程度、軀體疾病發(fā)生情況、精神殘疾發(fā)生情況、陽性癥狀評分、陰性癥狀評分、一般精神病理學癥狀評分、服藥依從性是精神分裂癥患者MOAS得分的影響因素(P0.05)。結論豐臺社區(qū)精神分裂癥患者的外顯攻擊行為發(fā)生率呈升高趨勢;教育程度低,存在軀體疾病、精神殘疾,伴有嚴重陽性癥狀、陰性癥狀及一般精神病理學癥狀,服藥依從性差是患者發(fā)生外顯攻擊行為的危險因素。提高患者衛(wèi)生服務條件、服藥依從性,指導患者按照合理劑量科學服藥,可在一定程度上避免外顯攻擊行為的發(fā)生。
[Abstract]:Objective to investigate the explicit aggressive behavior and influencing factors of schizophrenia in Fengtai district of Beijing. Methods A typical sampling method was used to select Fengtai community as the sample area. The archives of schizophrenic patients belonging to the community in Beijing Mental Health Information Management system (BMHIMS) from May 2011 to May 2015 were collected. Including demographic data, clinical information and behavior-related data. (MOAS) was used to evaluate the occurrence of explicit aggressive behavior. The total score = 0 was regarded as no explicit aggressive behavior, and the total score of 0 was the occurrence of explicit aggressive behavior. Results from 2011 to 2015, there was a significant difference in the prevalence of schizophrenia, the incidence of schizophrenia and the incidence of explicit aggressive behavior among the residents of Fengtai community (P0.05). Among them, the incidence of explicit aggression in schizophrenic patients increased from 27.1% in 2011 to 35.7% in 2015. In 2015, there were 616 BMHIMS schizophrenic patients in Fengtai community. Among them 220 cases (35.7%) had explicit attack behavior. The average age, education level, physical disease, mental disability, positive symptom score, negative symptom score, general psychopathological symptom score and drug compliance were compared in patients with explicit aggressive behavior. The difference was statistically significant (P0.05). Multiple linear regression analysis showed that education level, physical disease occurrence, mental disability, positive symptom score, negative symptom score, general psychopathology symptom score, Drug compliance was the influencing factor of MOAS score in schizophrenic patients (P0.05). Conclusion the incidence of explicit aggression in patients with schizophrenia in Fengtai community is increasing. Low education level, physical disease, mental disability, severe positive symptoms, negative symptoms and general psychopathological symptoms were the risk factors for explicit aggressive behavior. To improve patients' health service condition, drug compliance and guide patients to take medicine scientifically according to reasonable dosage can avoid the occurrence of explicit aggression to a certain extent.
