【摘要】：目的觀察阿爾茨海默病(AD)患者全腦三維(3D)偽連續(xù)動脈自旋標(biāo)記(3DpcASL)成像中腦血流灌注(CBF)特點,分析其與認知功能評分的相關(guān)性。方法對16例AD患者(AD組)和19例年齡匹配的認知功能正常者(對照組)進行常規(guī)MR和3DpcASL灌注成像。使用SPM 8軟件對所獲腦血流圖進行預(yù)處理,比較兩組CBF差異,評價CBF減低腦區(qū)CBF值與MMSE、MoCA評分的相關(guān)性。結(jié)果 AD組CBF灌注減低區(qū)主要見于雙側(cè)顳上回、顳中回、顳下回、雙側(cè)楔前葉、雙側(cè)中扣帶回、后扣帶回、左側(cè)角回、左側(cè)頂下回及左側(cè)額上回(t=5.32,P0.001);雙側(cè)楔前葉、雙側(cè)中后扣帶回、雙側(cè)顳上回、左側(cè)顳中回、左側(cè)顳下回、左側(cè)角回及左側(cè)頂下回CBF值與MMSE、MoCA評分呈正相關(guān)(P0.05)。結(jié)論3DpcASL可發(fā)現(xiàn)AD患者腦區(qū)灌注異常,灌注減低區(qū)的CBF值與認知功能減低存在相關(guān)性。
[Abstract]:Objective to observe the characteristics of cerebral blood flow perfusion (CBF) in three-dimensional (3D) pseudo-continuous arterial spin labeling (3DpcASL) imaging of patients with Alzheimer's disease (AD), and to analyze its correlation with cognitive function score. Methods conventional MR and 3DpcASL perfusion imaging were performed in 16 patients with AD (AD group) and 19 age-matched patients with normal cognitive function (control group). The SPM 8 software was used to pretreat the obtained cerebral blood flow map, and the difference of CBF between the two groups was compared, and the correlation between the CBF value of the reduced brain area of CBF and the MMSE,MoCA score was evaluated. Results in AD group, the areas of decreased CBF perfusion were mainly found in bilateral superior temporal gyrus, middle temporal gyrus, inferior temporal gyrus, bilateral precuneal lobe, bilateral middle cingulate gyrus, posterior cingulate gyrus, left angular gyrus, left inferior parietal gyrus and left superior frontal gyrus (t = 5.32, P0.001). The CBF values of bilateral anterior cuneate lobe, bilateral posterior cingulate gyrus, bilateral superior temporal gyrus, left middle temporal gyrus, left inferior temporal gyrus, left angular gyrus and left inferior parietal gyrus were positively correlated with MMSE,MoCA score (P0.05). Conclusion 3DpcASL can find abnormal cerebral perfusion in AD patients. The CBF value in hypoperfusion area is correlated with cognitive impairment.
