男性酒依賴患者戒斷期前瞻記憶研究

【摘要】:目的研究男性酒依賴患者戒斷期基于事件前瞻記憶(EBPM)和基于時(shí)間前瞻記憶(TBPM)的特征,明確基于事件前瞻記憶(EBPM)和基于時(shí)間前瞻記憶(TBPM)在男性酒依賴患者中的損害情況,探討前瞻記憶與臨床特征的關(guān)系,并對(duì)前瞻記憶與執(zhí)行功能的關(guān)系進(jìn)行分析。 方法選取32例男性酒依賴患者和30名年齡、受教育程度與酒依賴組相匹配的正常對(duì)照者,排除標(biāo)準(zhǔn)包括:神經(jīng)系統(tǒng)疾病者,既往有腦外傷病史及精神疾病史,嚴(yán)重軀體疾病及其他精神活性物質(zhì)濫用者,進(jìn)行前瞻記憶測(cè)試及與前額葉功能相關(guān)的執(zhí)行功能實(shí)驗(yàn)。 結(jié)果患者組的EBPM評(píng)分[(3.91±0.69)分]和TBPM評(píng)分[(3.28±0.77)分],與正常對(duì)照組[EBPM:(4.47±0.63)分、TBPM:(4.20±0.66)分]相比,差異有統(tǒng)計(jì)學(xué)意義(t=-3.348,P0.0l;t=-5.032,P0.0l);颊呓M在數(shù)字廣度實(shí)驗(yàn)的順向測(cè)試(t=-2.580,P=0.012)和反向測(cè)試成績(jī)均低于對(duì)照組(t=-5.032,P=0.005),,數(shù)字符號(hào)得分低于對(duì)照(t=-5.327,P<0.001)。與對(duì)照組相比,酒依賴患者Stroop色詞測(cè)驗(yàn)C部分反應(yīng)時(shí)(41.70±7.37)長(zhǎng)于對(duì)照組(33.33±6.64,P0.001)、正確閱讀數(shù)(21.13±1.74)低于對(duì)照組(22.47±1.28,P0.01);B部分反應(yīng)時(shí)(22.10±4.03)長(zhǎng)于對(duì)照組(19.30±3.18,P0.01),正確閱讀數(shù)差異無統(tǒng)計(jì)學(xué)意義(P0.05)。A部分反應(yīng)時(shí)、正確閱讀數(shù)差異均無統(tǒng)計(jì)學(xué)意義(P0.05),與對(duì)照組相比,酒依賴患者干擾效應(yīng)(19.62±6.97)高于對(duì)照組(13.70±5.69)(P0.01)。 相關(guān)分析顯示,患者EBPM評(píng)分、TBPM評(píng)分與飲酒時(shí)間、成癮時(shí)間、日飲酒量、數(shù)字廣度、數(shù)字符號(hào)無顯著相關(guān)性,與戒斷時(shí)間呈正相關(guān)(r=0.444,P0.05;r=0.423,P0.05),TBPM評(píng)分與Stroop色詞測(cè)試呈負(fù)相關(guān)(r=-0.479,P0.01)。 結(jié)論男性酒依賴患者戒斷期基于事件前瞻記憶和基于時(shí)間前瞻記憶均存在損害,患者戒斷期執(zhí)行功能存在損害,前瞻記憶與其戒斷時(shí)間存在相關(guān)性,基于時(shí)間前瞻記憶與Stroop實(shí)驗(yàn)中沖突抑制能力和Stroop效應(yīng)呈負(fù)相關(guān),因此,前額葉損害可能是戒斷期患者前瞻記憶損害的神經(jīng)機(jī)制之一。
[Abstract]:Objective to study the characteristics of event-based prospective memory (EBPM) and time-based prospective memory (TBPM) in male patients with alcohol dependence. To explore the relationship between prospective memory and clinical features and to analyze the relationship between prospective memory and executive function in male patients with alcohol dependence by determining the damage of event-based prospective memory (EBPM) and time-based prospective memory (TBPM). Methods 32 male patients with alcohol dependence and 30 healthy controls with education level matched with alcohol dependence group were selected. The exclusion criteria included: nervous system disease, previous history of brain trauma and mental illness. Patients with severe somatic diseases and other psychoactive substance abusers were tested for prospective memory and executive function related to prefrontal function. Results the scores of EBPM [(3.91 鹵0.69)] and TBPM [(3.28 鹵0.77)] in the patients group were compared with those in the control group [EBPM: (4.47 鹵0.63), TBPM: (4.20 鹵0.66]. The difference was statistically significant (t = 3.348, P 0.0l). T0. 01- 5. 032g. P 0. 0l). The scores of forward test and reverse test in the digital span test were lower than those in the control group (t = -5.032, P < 0.005), and the scores of digital symbols were lower than those in the control group (t = -5.327, P < 0.001). Compared with the control group, the C part reaction time of Stroop color words test in the patients with alcohol dependence was (41.70 鹵7.37) longer than that in the control group (33.33 鹵6.64 P0.001), and the correct reading number was (21.13 鹵1.74) lower than that in the control group (22.47 鹵1.28 P0.01). Part B reaction time (22.10 鹵4.03) was longer than that of control group (19.30 鹵3.18 P0.01), and there was no significant difference in correct reading number (P0.05). Compared with the control group, the interference effect of the patients with alcohol dependence (19.62 鹵6.97) was higher than that of the control group (13.70 鹵5.69) (P0.01). Correlation analysis showed that there was no significant correlation between EBPM score, TBPM score and drinking time, addiction time, daily alcohol consumption, digital span, digital symbol, but positively correlated with withdrawal time (r = 0.444p 0.05). There was a negative correlation between Stroop color word test and), TBPM score (r = 0.423, P 0.01). Conclusion both event-based prospective memory and time-based prospective memory are impaired in the male patients with alcohol dependence, and the executive function of the patients is impaired during the withdrawal period, and prospective memory is correlated with the withdrawal time. Temporal prospective memory is negatively correlated with the ability of conflict inhibition and Stroop effect in Stroop experiment. Therefore, prefrontal lobe damage may be one of the neural mechanisms of prospective memory impairment in abstinent patients.
