【摘要】：目的：觀察高壓氧結(jié)合針刺治療卒中后抑郁的臨床療效。 方法：將60例符合納入標(biāo)準(zhǔn)的卒中后抑郁住院患者隨機(jī)分為高壓氧結(jié)合針刺組和對(duì)照組,高壓氧結(jié)合針刺組和對(duì)照組各30例。高壓氧結(jié)合針刺組：男性12例,女性18例；年齡最大的80歲,年齡最小的45歲,平均年齡62.10±8.99歲；對(duì)照組：男性14例,女性16例；年齡最大的78歲,年齡最小的42歲,平均年齡62.03±9.67歲。兩組患者均給予神經(jīng)內(nèi)科常規(guī)處理(控制血壓,調(diào)節(jié)血糖等)及針刺治療,高壓氧結(jié)合針刺組同時(shí)加用高壓氧治療。高壓氧每天1次,每次120min,壓力為2ATA；針刺采用“健腦調(diào)神”法：穴取大椎、四神聰、上星,鳩尾每日1次,每次30min,10次為一個(gè)療程,連續(xù)治療兩個(gè)療程。兩組患者在治療前和治療20天后均采用漢密爾頓抑郁量表(Hamilton Dpression, HAMD)和日常生活活動(dòng)能力量表(Activity of Daily Living Scale, ADL)進(jìn)行療效評(píng)定,所得數(shù)據(jù)用SPSS18.0軟件進(jìn)行統(tǒng)計(jì)學(xué)分析。 結(jié)果：1.高壓氧結(jié)合針刺組患者治療前后HAMD量表評(píng)分有明顯差異(P0.01),提示高壓氧結(jié)合針刺對(duì)于改善卒中后抑郁患者的抑郁癥狀有較好的療效；2.高壓氧結(jié)合針刺組患者治療前后ADL量表評(píng)分有明顯差異(P0.01),提示針刺對(duì)于改善卒中后抑郁患者的日常生活能力有較好的療效；3.高壓氧結(jié)合針刺組與對(duì)照組組間療效比較,兩組相比有差異(P0.05),說(shuō)明高壓氧結(jié)合針刺組可以取得更好的療效。 結(jié)論：1.高壓氧結(jié)合針刺治療對(duì)改善卒中后抑郁可以取得較好的療效,可明顯改善患者的抑郁狀態(tài)以及提高日常生活能力。2.高壓氧結(jié)合針刺法是值得在治療卒中后抑郁臨床中推廣應(yīng)用的。
[Abstract]:Objective: to observe the clinical effect of hyperbaric oxygen combined with acupuncture on post-stroke depression. Methods: sixty inpatients with post-stroke depression were randomly divided into hyperbaric oxygen combined with acupuncture group and control group, and hyperbaric oxygen combined with acupuncture group and control group with 30 cases each. Hyperbaric oxygen combined with acupuncture group: male 12, female 18; oldest 80 years, the youngest 45 years, mean age 62.10 鹵8.99; control group: 14 males, 16 females, the oldest 78 years, the youngest 42 years old. The average age was 62.03 鹵9.67 years old. Patients in both groups were given routine treatment (blood pressure control, blood glucose regulation, etc.) and acupuncture treatment. Hyperbaric oxygen combined with acupuncture group was also treated with hyperbaric oxygen. The pressure of hyperbaric oxygen was 120 min, the pressure was 2ATA.Acupuncture was done by "invigorating the brain and regulating the mind": the acupoints were collected from Dazhui, Sishen Cong, Star and Dove tail once a day, 30 minutes and 10 times a day as a course of treatment for two consecutive courses of treatment. Both groups were evaluated with Hamilton Depression scale (Hamilton Dpression, HAMD) and activity of Daily living scale (Activity of Daily Living Scale, ADL) before treatment and 20 days after treatment. The data were analyzed by SPSS18.0 software. The result is 1: 1. The scores of HAMD scale were significantly different before and after treatment in hyperbaric oxygen combined with acupuncture group (P0.01), which suggested that hyperbaric oxygen combined with acupuncture had a better effect on improving depression symptoms of post-stroke depression patients (P0.01). The scores of ADL scale in hyperbaric oxygen combined with acupuncture group were significantly different before and after treatment (P0.01), which suggested that acupuncture had a better effect on improving ADL of patients with post-stroke depression (P0.01). Hyperbaric oxygen combined with acupuncture group and the control group, there was a difference between the two groups (P0.05), indicating that the hyperbaric oxygen combined with acupuncture group can achieve a better effect. Conclusion 1. Hyperbaric oxygen combined with acupuncture can improve the depression after stroke, improve the depression state of patients and improve the ability of daily living (ADL). Hyperbaric oxygen combined with acupuncture is worth popularizing in the treatment of post-stroke depression.
