【摘要】：目的:采用靜息狀態(tài)下血氧水平依賴功能磁共振(BOLD-fMRI)技術(shù)和低頻振幅(ALFF)及比率低頻振幅(fALFF)方法觀察小腦頂核電刺激對(duì)阿爾茨海默病(AD)神經(jīng)元活動(dòng)性的影響。方法:將36例AD患者隨機(jī)分為兩組:治療組18例,安慰治療組18例,給予小腦頂核電刺激(FNS)治療。治療前后分別行靜息態(tài)fMRI掃描,應(yīng)用ALFF及fALFF方法分析腦功能區(qū)變化情況,采用統(tǒng)計(jì)學(xué)方法,比較治療前后的差異。并且分別將治療后的ALFF圖及fALFF圖與神經(jīng)心理學(xué)檢查量表評(píng)分作相關(guān)性分析。結(jié)果:FNS治療后治療組左側(cè)小腦后葉、右側(cè)小腦后葉、左側(cè)海馬/海馬旁回、左側(cè)后扣帶回、左側(cè)背外側(cè)前額葉及右側(cè)頂下小葉ALFF值升高,右側(cè)頂下小葉fALFF值升高。安慰治療組治療后雙側(cè)小腦后葉及左側(cè)后扣帶回的ALFF值較治療前降低。ADAS-Cog評(píng)分與治療后左側(cè)小腦后葉及右側(cè)小腦后葉的ALFF值密切相關(guān)。結(jié)論:FNS能夠改變AD患者部分腦區(qū)神經(jīng)元的活動(dòng)性。BOLD-fMRI信號(hào)的改變可為探索小腦頂核電刺激治療AD的中樞機(jī)制提供潛在的影像學(xué)證據(jù)。
[Abstract]:Aim: to investigate the effects of apical nuclear stimulation on the activity of (AD) neurons in Alzheimer's disease using resting oxygen level dependent functional magnetic resonance (BOLD-fMRI) technique, low frequency amplitude (ALFF) and ratio low frequency amplitude (fALFF). Methods: 36 patients with AD were randomly divided into two groups: treatment group (n = 18) and consolation group (n = 18). The patients were treated with cerebellar fastigial nuclear stimulation (FNS). Resting fMRI scanning was performed before and after treatment. The changes of brain functional areas were analyzed by ALFF and fALFF. The differences before and after treatment were compared by statistical method. The correlation between ALFF and fALFF after treatment and the scores of neuropsychological examination scale were analyzed. Results the ALFF values of left posterior cerebellar lobe, right posterior cerebellar lobe, left hippocampal / parahippocampal gyrus, left posterior cingulate gyrus, left dorsolateral prefrontal lobe and right inferior parietal lobule increased in the treatment group. The ALFF scores of bilateral posterior cerebellar lobes and left posterior cingulate gyrus in the comfort treatment group were significantly lower than those before treatment. The ADAS-Cog score was closely related to the ALFF value of the left posterior cerebellar lobe and the right posterior cerebellar lobe. Conclusion the changes of brain neuronal activity. BOLD-fMRI signal can provide potential imaging evidence for exploring the central mechanism of cerebellar fastigial nuclear stimulation in the treatment of AD.
