【摘要】：目的探討不同戒斷時(shí)間對(duì)海洛因成癮者大腦靜息態(tài)下功能網(wǎng)絡(luò)的影響。方法 16名強(qiáng)制戒斷11~13個(gè)月的海洛因成癮者(PA12組)、20名強(qiáng)制戒斷5~7個(gè)月的海洛因成癮者(PA6組)納入靜息態(tài)fMRI研究。運(yùn)用圖論理論構(gòu)建小世界腦網(wǎng)絡(luò),比較兩組海洛因成癮者小世界特性及核心節(jié)點(diǎn)特性,分析核心節(jié)點(diǎn)與戒斷時(shí)間的相關(guān)性。結(jié)果兩組腦網(wǎng)絡(luò)均具有小世界特性(γ≈1、λ?1)且差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);PA12組較PA6組腦網(wǎng)絡(luò)的左側(cè)中央前回及左側(cè)海馬旁回節(jié)點(diǎn)介數(shù)值降低,左側(cè)楔葉、左側(cè)顳極及右側(cè)枕中回節(jié)點(diǎn)介數(shù)值升高(P均0.05)。左側(cè)中央前回(r=—0.52,P=0.001)、左側(cè)海馬旁回(r=—0.49,P=0.002)節(jié)點(diǎn)介數(shù)值均與戒斷時(shí)間呈負(fù)相關(guān),右側(cè)枕中回節(jié)點(diǎn)介數(shù)值與戒斷時(shí)間呈正相關(guān)(r=0.49,P=0.003)。結(jié)論戒斷5~7個(gè)月后,海洛因成癮者大腦網(wǎng)絡(luò)小世界拓?fù)浣Y(jié)構(gòu)趨于穩(wěn)定;長(zhǎng)期戒斷有助于降低成癮者對(duì)毒品的相關(guān)記憶和潛在覓藥行為,恢復(fù)視覺(jué)空間注意能力。
