蝦青素對(duì)膿毒癥大鼠應(yīng)激性高血糖及血清脂聯(lián)素的影響

【摘要】:目的:通過構(gòu)建膿毒癥模型來探討1.蝦青素對(duì)膿毒癥大鼠存活率的影響2.蝦青素對(duì)膿毒癥大鼠應(yīng)激性高血糖及血清脂聯(lián)素的影響;3.蝦青素對(duì)膿毒癥大鼠心、肝、肺、腎等臟器的保護(hù)作用。 研究方法:采用盲腸結(jié)扎穿刺法(CLP)制備膿毒癥模型。1.48只8周齡雄性SD大鼠隨機(jī)分為4組(n=12):假手術(shù)組、膿毒癥組、膿毒癥+橄欖油組、膿毒癥+蝦青素組,觀察7天內(nèi)各組大鼠的存活率。2.32只8周齡雄性SD大鼠隨機(jī)分為4組(n=8):假手術(shù)+橄欖油組、假手術(shù)+蝦青素組、膿毒癥+橄欖油組、膿毒癥+蝦青素組。假手術(shù)組只接受剖腹和盲腸探查術(shù),膿毒癥組制作CLP模型。于術(shù)前、術(shù)后、6h、12h、24h尾靜脈采血測定快速血糖。術(shù)后24h經(jīng)實(shí)施腹腔葡萄糖耐量實(shí)驗(yàn),測得30min,60min,120min快速血糖及血清胰島素水平;留取12h及48h血清標(biāo)本測定脂聯(lián)素值。48h后處死大鼠取其心、肝、肺、腎組織行H-E染色,光鏡查看組織形態(tài)學(xué)變化。 結(jié)果:1.膿毒癥組及膿毒癥+橄欖油組7天存活率均為33.33%,顯著低于假手術(shù)組100%及膿毒癥+蝦青素組75%,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。2.術(shù)后4組血糖較術(shù)前均出現(xiàn)明顯升高(P0.01),組間無差異;術(shù)后6h,12h,24h膿毒癥+橄欖油組血糖均高于其他三組(P0.05)。3.腹腔注射葡萄糖后,各組血糖在30min升高最明顯,隨后降低,在30min,60min,120min膿毒癥+橄欖油組血糖均高于膿毒癥+蝦青素組(P0.05);四組血清胰島素水平在腹腔注射葡萄糖后開始升高,并隨時(shí)間遞增,膿毒癥+蝦青素組在各時(shí)間點(diǎn)血清胰島素水平均高于膿毒癥+橄欖油組(P0.05)。4.48h各組血清脂聯(lián)素濃度均低于12h,膿毒癥組較假手術(shù)組降低更明顯(P0.05),膿毒癥+蝦青素組在12h、48h均高于膿毒癥+橄欖油組(P0.05)。5.膿毒癥+蝦青素組光鏡下心、肝、肺、腎組織炎癥細(xì)胞浸潤較膿毒癥+橄欖油組明顯減輕,組織結(jié)構(gòu)相對(duì)完整。 結(jié)論:蝦青素能增加血清胰島素及脂聯(lián)素濃度,從而改善膿毒癥大鼠應(yīng)激性高血糖,亦對(duì)心、肝、肺、腎等臟器具有一定保護(hù)作用并能提高膿毒癥大鼠的存活率。
