急診急性心肌梗死、主動脈夾層患者血清DD、CRP、cTnI及血壓不對稱情況比較

【摘要】:目的探討急性心肌梗死、主動脈夾層患者急診入院時血清D二聚體(DD)、C反應蛋白(CRP)、肌鈣蛋白I(cTnI)水平及血壓不對稱情況的差異。方法回顧102例因6 h內急性胸(背)痛伴或不伴呼吸困難初次就診患者的臨床資料,其中72例最終診斷為急性心肌梗死(AMI組),30例最終診斷為主動脈夾層(AAD組)。入院后立即抽取靜脈血,采用乳膠法檢測血清DD,采用免疫散射比濁法檢測CRP,采用化學發(fā)光法檢測cTnI,測量兩臂血壓觀察是否存在血壓不對稱(雙臂收縮壓差≥20 mm Hg)。比較兩組血清DD、CRP水平及血清cTnI升高、血壓不對稱發(fā)生率。結果 AMI、AAD組患者血清DD水平分別為(362±141)、(1 608±546)μg/L,兩組相比P0.05。AMI、AAD組患者血清CRP水平分別為(12.9±3.60)、(13.8±3.48)mg/L,兩組差異無統(tǒng)計學意義。AMI組72例(100%)血清cTnI升高、1例(1.4%)血壓不對稱,AAD組4例(13.3%)血清cTnI升高、10例(33.3%)血壓不對稱,兩組血清cTnI升高、血壓不對稱發(fā)生率相比P0.05。結論急性心肌梗死、主動脈夾層患者急診入院時血清DD、cTnI和血壓不對稱情況存在差異。血清DD、cTnI可用于急診鑒別診斷急性心肌梗死和主動脈夾層。
