顱腦損傷早期外周血中性粒細(xì)胞氧化爆發(fā)和吞噬功能的研究

【摘要】:目的:研究顱腦損傷患者早期外周血中性粒細(xì)胞的氧化爆發(fā)及吞噬功能變化。 方法:選擇12例顱腦損傷患者(顱腦損傷組,TBI),12例骨折患者(創(chuàng)傷控制組,TC)及10例健康者(健康對照組,U),采用流式細(xì)胞術(shù)、Westernblot等方法分別于傷后6h、12h、24h、48h、72h、1w及2w檢測外周血中性粒細(xì)胞氧化爆發(fā)強度,吞噬率,自由基,NF-kB,gp91phox,COX-2,iNOS,及血漿TNF-α,IL-6和CRP的變化情況。 結(jié)果:TBI組較U組在各時間點中性粒細(xì)胞氧化爆發(fā)明顯增強(P<0.05),,且高于TC組;TBI組中性粒細(xì)胞的吞噬率在各時間點均明顯降低,而TC組在24h-72h降低(P<0.05),TC組中吞噬率在傷后1w可恢復(fù)正常范圍,TBI則在傷后2w仍處低水平吞噬活性狀態(tài);除外TC組在2w時,其余時間點TC和TBI組中性粒細(xì)胞自由基均明顯增加(P<0.05),且顱腦損傷越重,增加越明顯;傷后24hTBI組和TC組中g(shù)p91phox,COX-2和iNOS的表達(dá)均增加(P<0.05),且TBI組較TC組增加更顯著,而NF-kB的增加則兩者相似;細(xì)胞因子TNF-α,IL-6以及CRP在TC組1w后可趨于正常,而在TBI后2w內(nèi)仍處于高水平(P<0.01)。 結(jié)論:顱腦損傷早期外周血中性粒細(xì)胞氧化爆發(fā)作用增強,而吞噬功能下降。
[Abstract]:Objective: to study the oxidative burst and phagocytosis of peripheral neutrophils in patients with craniocerebral injury. Methods: twelve patients with craniocerebral injury (12 patients with, TBI), fracture in craniocerebral injury group and 10 healthy subjects with, TC) in trauma control group) were treated with flow cytometry and Westernblot respectively at 6 hours after injury, 12 hours after injury and 24 hours after injury. The oxidative burst intensity, phagocytosis rate, free radical, NF-kB,gp91phox,COX-2,iNOS, plasma TNF- 偽, IL-6 and CRP of peripheral blood neutrophils were measured for 1 week and 2 weeks. Results: the oxidative burst of neutrophil in TBI group was significantly higher than that in U group at each time point (P < 0. 05), and higher than that in TC group. The phagocytosis rate of neutrophils in TBI group was significantly decreased at each time point, while that in TC group was decreased in 24h-72h group (P < 0. 05). The phagocytosis rate of TC group returned to normal at 1 week after injury, while the phagocytic activity of TBI was still low at 2 weeks after injury. The neutrophil free radicals in TC and TBI groups increased significantly at the other time points (P < 0. 05), and the more severe the craniocerebral injury, the more obvious the increase. The expression of gp91phox,COX-2 and iNOS in 24hTBI group and TC group were increased after injury (P < 0. 05), and the expression of gp91phox,COX-2 and iNOS in TBI group was significantly higher than that in TC group, but the increase in NF-kB was similar. The cytokines TNF- 偽, IL-6 and CRP tended to be normal after 1 week in TC group, but remained at high level within 2 weeks after TBI (P < 0. 01). Conclusion: in the early stage of craniocerebral injury, the oxidative burst of peripheral blood neutrophils is enhanced, but the phagocytosis is decreased.
