【摘要】：目的評(píng)價(jià)豬尾導(dǎo)管與普通胸管用于氣胸引流治療的有效性及安全性。方法通過(guò)計(jì)算機(jī)檢索Pub Med、Cochrane圖書(shū)館、Embase、谷歌學(xué)術(shù)等英文數(shù)據(jù)庫(kù)截至2015年11月26日有關(guān)豬尾導(dǎo)管與普通胸管治療氣胸的臨床對(duì)照試驗(yàn),使用MINORS條目對(duì)納入文獻(xiàn)的方法學(xué)質(zhì)量進(jìn)行評(píng)價(jià),采用Revman Manager 5.1軟件進(jìn)行Meta分析。結(jié)果共納入4篇文獻(xiàn),包括1篇隨機(jī)對(duì)照研究、3篇非隨機(jī)對(duì)照試驗(yàn),共522例患者,其中使用豬尾導(dǎo)管267例、使用普通胸管255例。經(jīng)過(guò)Meta分析:豬尾導(dǎo)管組相對(duì)于普通胸管組,減少了術(shù)后置管時(shí)間(WMD=-0.33,95%CI-0.58~-0.07,P0.05),而氣胸治療成功率(RR=0.94,95%CI 0.87~1.02,P0.05)、術(shù)后住院時(shí)間(WMD=-2.33,95%CI-6.87~2.22,P0.05)、相應(yīng)并發(fā)癥的發(fā)生率(RR=0.73,95%CI 0.29~1.86,P0.05),差異均無(wú)統(tǒng)計(jì)學(xué)意義。結(jié)論相比于普通胸管,應(yīng)用豬尾導(dǎo)管治療氣胸,可有效減少患者術(shù)后置管時(shí)間,而并發(fā)癥、術(shù)后住院時(shí)間、成功率差異均無(wú)統(tǒng)計(jì)學(xué)意義。豬尾導(dǎo)管可有效用于氣胸患者的胸腔閉式引流。
[Abstract]:Objective to evaluate the efficacy and safety of pig tail catheter and common chest tube in the treatment of pneumothorax drainage. Methods Pub Med,Cochrane library, Embase, Google academic database and other English databases were searched by computer. As of November 26, 2015, the clinical controlled trial of pig tail catheter and common chest tube in treating pneumothorax was carried out. MINORS items were used to evaluate the methodological quality of the literature, and Revman Manager 5.1 software was used for Meta analysis. Results A total of 4 articles were included, including 1 randomized controlled study and 3 non-randomized controlled trials. A total of 522 patients were included, 267 of whom were treated with pig tail catheter and 255 with common thoracic duct. After Meta analysis, compared with the common chest tube group, the pig tail catheter group reduced the postoperative catheter placement time (WMD=-0.33,95%CI-0.58~-0.07,P0.05), and the success rate of pneumothorax treatment (RR=0.94,95%CI 0.871.02). There was no significant difference in postoperative hospitalization time (WMD=-2.33,95%CI-6.87~2.22,P0.05) and the incidence of complications (RR=0.73,95%CI 0.29, 1.86 P 0.05). Conclusion compared with the common chest tube, the treatment of pneumothorax with pig tail catheter can effectively reduce the time of catheterization, but there is no significant difference in complications, postoperative hospitalization time and success rate of pneumothorax. Pig tail catheter can be effectively used for thoracic closed drainage in patients with pneumothorax.
