封閉負壓引流技術促進慢性創(chuàng)面愈合機制的研究

【摘要】:目的研究慢性創(chuàng)面中表皮生長因子(epidermal growth factor,EGF)、血小板衍化生長因子(platelet derived growth factor,,PDGF)、血管內皮細胞生長因子(vascular endothelialgrowth factor,VEGF)等生長因子及其受體表達的變化,探討封閉負壓引流技術(vacuum sealing drainage,VSD)促進慢性創(chuàng)面愈合的機制。 方法選取我院2011年1月-2012年12月慢性創(chuàng)面患者40例,隨機分為治療組和對照組,每組20人。治療組創(chuàng)面用封閉負壓引流治療,對照組創(chuàng)面用傳統(tǒng)換藥治療。兩組均在治療前及治療后1周取創(chuàng)面組織5mm×5mm。采用光鏡、免疫組化等方法,觀察創(chuàng)面愈合過程中EGF、PDGF及VEGF等生長因子及其受體的表達的變化。 結果治療組病人創(chuàng)面肉芽組織比較新鮮,血供豐富,而對照組病人創(chuàng)面則無明顯變化。光鏡見治療組治療后1周創(chuàng)面組織切片中藍染部分明顯比對照組明顯增加。免疫組化見治療組創(chuàng)面組織中EGF、PDGF及VEGF及其受體表達明顯比對照組增多,且兩者差異均有統(tǒng)計學意義(P0.05)。 結論封閉負壓引流能改善創(chuàng)面血液循環(huán),促進創(chuàng)面肉芽組織生長,增加創(chuàng)面組織中EGF、PDGF、VEGF等生長因子及其受體的表達,從而加速創(chuàng)面愈合。
[Abstract]:Objective to study the expression of epidermal growth factor (epidermal growth), platelet-derived growth factor (platelet derived growth) and vascular endothelial growth factor (vascular endothelialgrowth) in chronic wounds. Objective: to investigate the mechanism of (vacuum sealing drainage (VSD) in promoting the healing of chronic wounds. Methods from January 2011 to December 2012, 40 patients with chronic wounds in our hospital were randomly divided into treatment group and control group with 20 patients in each group. The wounds in the treatment group were treated with closed negative pressure drainage, while the wounds in the control group were treated with traditional dressing change. 5mm 脳 5 mm of wound tissue were taken before and 1 week after treatment in both groups. The expression of EGF- PDGF and VEGF and their receptors during wound healing were observed by light microscopy and immunohistochemistry. Results the granulation tissue was fresh and blood supply was abundant in the treatment group, but there was no obvious change in the control group. The blue staining in the tissue sections of the wounds in the treatment group was significantly higher than that in the control group 1 week after treatment. Immunohistochemistry showed that the expression of EGF- PDGF, VEGF and its receptor in the wound tissue of the treatment group was significantly higher than that of the control group, and the difference between the two groups was statistically significant (P0.05). Conclusion blocking negative pressure drainage can improve the blood circulation of wound, promote the growth of granulation tissue, increase the expression of VEGF and its receptor in wound tissue, and accelerate wound healing.
