【摘要】：目的探討導(dǎo)致急性期腦梗死后遠(yuǎn)隔損害的影像及臨床表現(xiàn)相關(guān)預(yù)測(cè)因素。方法分析2014年6月至12月在山東大學(xué)第二醫(yī)院住院治療的連續(xù)納入的單側(cè)大腦中動(dòng)脈供血區(qū)梗死患者106例臨床資料。按梗死后14 d是否出現(xiàn)遠(yuǎn)隔損害影像學(xué)表現(xiàn)分為陽(yáng)性組(39例)與陰性組(67例),進(jìn)行單因素相關(guān)分析,并繼續(xù)追蹤其梗死后3個(gè)月臨床結(jié)局。結(jié)果影像學(xué)陽(yáng)性患者39例,陽(yáng)性率36%。陽(yáng)性組其3個(gè)月臨床結(jié)局差于陰性組(mRS評(píng)分,U=1 446.500,P=0.022);單因素分析顯示,陽(yáng)性組入院時(shí)美國(guó)國(guó)立衛(wèi)生研究院卒中量表(NIHSS)評(píng)分高于陰性組(U=960.000,P0.01),陽(yáng)性組早期癥狀改善率低于陰性組(24 h NIHSS評(píng)分降低≥4分或降至0分;χ2=19.758,P0.01)。結(jié)論入院時(shí)NIHSS評(píng)分和早期癥狀改善是急性期腦梗死后遠(yuǎn)隔損害的相關(guān)因素。
[Abstract]:Objective to investigate the imaging and clinical manifestations of distant lesions after acute cerebral infarction. Methods the clinical data of 106 consecutive patients with unilateral middle cerebral artery (MCAA) infarction who were hospitalized in the second Hospital of Shandong University from June to December 2014 were analyzed. According to the imaging findings of distant lesion 14 days after infarction, 39 cases were divided into positive group (39 cases) and negative group (67 cases). Univariate correlation analysis was carried out, and the clinical outcome was continued to be tracked 3 months after infarction. Results 39 cases were positive in imaging, the positive rate was 36%. The clinical outcome of the positive group was worse than that of the negative group in 3 months. At admission, the NIHSS score of the positive group was higher than that of the negative group, and the improvement rate of the early symptoms in the positive group was lower than that in the negative group at 24 h or more than that in the negative group. Conclusion the NIHSS score and the improvement of early symptoms on admission are related factors of distant lesion after acute cerebral infarction.
