【摘要】：目的研究早期腸內(nèi)營養(yǎng)(EEN)對重癥急性胰腺炎(SAP)患者血清C反應(yīng)蛋白(CRP)及T淋巴細胞亞群的影響。方法將45例SAP患者隨機分為實驗組(22例)和對照組(23例)。實驗組給予EEN,對照組給予腸外營養(yǎng)(PN),待患者排氣排便后給予EN(通常于入院后5～7 d)。于入院后第1、5、7、11天,檢測兩組患者血清CRP及T淋巴細胞亞群(CD3、CD4+、CD4+/CD8+值)。結(jié)果除3例排除實驗,余42例SAP患者治愈。平均住院日及住院費用,實驗組低于對照組(P0.05)。兩組間比較,血清CRP、T淋巴細胞亞群,在第1天的差異無統(tǒng)計學意義(P0.05);第5、7、11天血清CRP水平,實驗組低于對照組(P0.05),T淋巴細胞亞群(CD3、CD4+、CD4+/CD8+值)水平實驗組高于對照組(P0.05)。結(jié)論給予EEN能更好地降低SAP患者急性反應(yīng)期炎癥反應(yīng),提高免疫功能。
[Abstract]:Objective to study the effect of early enteral nutrition (EEN) on serum C-reactive protein (CRP) and T lymphocyte subsets in patients with severe acute pancreatitis (SAP). Methods 45 patients with SAP were randomly divided into experimental group (n = 22) and control group (n = 23). The experimental group was given EEN, the control group was given parenteral nutrition (PNN), and the patient was given en after exhaust defecation (usually 5 days after admission). The serum CRP and T lymphocyte subsets (CD3T) CD4 / CD 4 / CD 8 were measured on the 7th and 11th day after admission. Results 42 cases of SAP were cured except 3 cases of exclusion test. The average days of hospitalization and the cost of hospitalization in the experimental group were lower than those in the control group (P 0.05). There was no significant difference between the two groups in serum CRP T lymphocyte subsets on the first day (P 0.05), and the level of serum CRP was lower in the experimental group than that in the control group on day 711, and the level of CD4 / CD8 in the experimental group was higher than that in the control group (P 0.05). Conclusion EEN can decrease inflammatory reaction and improve immune function in patients with SAP.
