【摘要】：目的探討Penumbra抽吸系統(tǒng)聯(lián)合Trevo取栓器治療顱內(nèi)大動(dòng)脈急性閉塞的臨床效果。方法收集2016年11月至12月采用ACETM取栓器聯(lián)合Trevo可視性取栓支架機(jī)械取栓治療的5例顱內(nèi)大動(dòng)脈急性梗死患者臨床資料。其中前循環(huán)閉塞2例,后循環(huán)閉塞3例;平均(60.4±11.6)歲。術(shù)后觀察患者取栓時(shí)間、閉塞血管再通和神經(jīng)功能恢復(fù)情況。結(jié)果 5例患者穿刺-血管再通時(shí)間分別為29 min、32 min、35 min、33 min、30 min,平均(31.8±2.4)min;術(shù)中腦梗死溶栓(TICI)治療后血流分級(jí)均達(dá)到3級(jí);NIHSS評(píng)分由術(shù)前(11.0±7.4)分明顯改善至術(shù)后24 h(4.2±1.1)分、7 d(1.8±1.3)分、30 d(0.9±0.6)分(P0.05);改良Rankin量表(m RS)評(píng)分均為0~2分,且未發(fā)生顱內(nèi)出血轉(zhuǎn)化。結(jié)論 ACETM取栓器聯(lián)合Trevo取栓支架機(jī)械取栓治療顱內(nèi)大動(dòng)脈急性梗死,在取栓次數(shù)少、取栓時(shí)間短情況下,取得了最佳血管再通效果,臨床預(yù)后良好。
[Abstract]:Objective to investigate the clinical effect of Penumbra aspiration system combined with Trevo thrombectomy in the treatment of acute intracranial artery occlusion. Methods from November to December 2016, the clinical data of 5 patients with acute Intracranial Arterial Infarction treated with ACETM thrombectomy combined with Trevo visible thrombectomy stents were collected. There were 2 cases of anterior circulation occlusion and 3 cases of posterior circulation occlusion, with an average of 60.4 鹵11.6 years old. Postoperative thrombectomy time, recanalization of occluded vessels and recovery of nerve function were observed. Results the mean time of puncture and vascular recanalization was 29 min / 32 min / 35 min / 33 min / 30 min, with an average of 31.8 鹵2.4 min, respectively, and the blood flow grade reached grade 3 after thrombolytic therapy (from 11.0 鹵7.4) to 24 h (4.2 鹵1.1) / 7 d (1.8 鹵1.3) and 30 min respectively. The scores of the modified Rankin scale and the modified Rankin scale were all 0 ~ 2 points, P < 0. 05, 0. 9 鹵0. 6) and 0 ~ 2, respectively. No intracranial hemorrhage occurred. Conclusion ACETM thrombectomy combined with Trevo stent mechanical thrombectomy in the treatment of acute cerebral artery infarction, under the condition of less thrombus removal times and shorter thrombectomy time, the best vascular recanalization effect is obtained, and the clinical prognosis is good.
